Extension of BRMSD's existing Low Pressure Sewer Collection System with
construction of approximately 105,000 linear feet of 2", 3", 4", 6", and 8" PVC low
pressure sewer main.
Construction of 16,000 linear feet of 8" transmission force main between the
Grantsboro and Bayboro.
Construct new septic tank effluent pumping systems to serve 638 properties within
the proposed Reelsboro Sewer Service Area.
Two new 4" and one new 12" HDPE directional drill creek crossings in the NCDOT
right-of-way along SR 1125 (Bennett Farm Road) and NC Highway 55.
A CAMA General Permit was previously applied for on February 9, 2000 for the proposed
crossing but was pending due to failure to receive final verification that all adjacent property
owners were notified. Funding has been awarded for the construction of the project and the
CAMA General Permit must be reapplied for due to fact that the previous submittal is over a
year old.
We have re -contacted landowners adjacent to the proposed crossings via certified mail to notify
them of our project and to request public comment. Certified letters were sent to six landowners
(refer to attached documents for names and mailing addresses). We received return receipts
from the certified mailings on May 22nd and written or verbal responses from four landowners
(attached) since that time. The mandatory 10 day public comment period ended on May 15,
2001.

Since the mandatory 10 -day public review period has now expired and we have certified
receipts from all the appropriate landowners, we are officially requesting a CAMA General
Permit for the proposed sewer collection system improvements and have included the Permit
Application Fee of $100.00.
Minor design changes have been made to the plans since the notifications were mailed to the
adjacent landowners. The changes at the jurisdictional crossings include:
The crossing at Goose Creek along NC 55 will be made five feet inside the south side
right of way and will be a 600 LF HDPE directional drill crossing. The route change was
made in response to design coordination with NCDOT. The pressure sewer main was
relocated to the south side of the roadway to avoid conflict with proposed roadway
improvements to NC 55 for NCDOT project R 25398.
The HDPE directional drill crossing at West Fork Goose Creek along SR 1125 (Bennett
Farm Rd.) will be 220 LF of 4" HDPE pipe. Previously, this segment of pipe was
designed as 180 LF of 3" HDPE pipe.
The HDPE directional drill crossing at the Unnamed Tributary of West Fork Goose Creek
along SR 1125 (Bennett Farm Rd.) will be 200 LF of 4" HDPE pipe. Previously, this
segment of pipe was designed as 160 LF of 3" HDPE pipe.

We have contacted the US Army Corp of Engineers to notify them of the crossings where
wetland and stream impacts may occur. All other necessary permit request including Erosion
and Sedimentation Control, NCDOT Encroachment, and Pressure Sewer Systems are being
filed at this time.
To assist you in your review, we are attaching a copy of the USGS quad maps showing the
location of the proposed creek crossings, copies of construction plans for the directional drill
crossings including the revised plan sheets for the changes discussed above, and evidence of
the required property owner notifications.
Please review this information and if acceptable, issue a new CAMA General Permit for this

informed Chris that the application is in order, but that you cannot issue a permit until you
received a letter verifying that the BRMSD Bayboro WWTF has adequate reserve treatment
capacity from NC DENR. I have been in contact with Mr. Roger Thorpe concerning this issue
and he recommended that I send you a copy of the recently issued DENR WQ permit that
verifies that NCDENR has reviewed and approved the project. To this end, I am attaching a
copy of the permit for your review.

construction and operation of approximately 25,375 linear feet of 8 -inch, 18,520 linear feet of 6 -
inch, 18,565 linear feet of 4 -inch, 17,300 linear feet of 3 -inch and 7,600 linear feet of 2 -inch
pressure sewer main; approximately 31,145 linear feet of 1 -inch, 3,495 linear feet of 1.25 -inch,
and 2,150 linear feet of 1.5 -inch of pressure service line; directional bore with 12 -inch pipe for
Goose Creek crossing; directional bore with 4 -inch pipe for Moore's Swamp crossing, West
Goose Creek crossing and Bennett Farm Road crossing; construction of 250 S.T.E.P. systems;
associated site work, and appurtenances to serve the Western Bay River Metropolitan Sewerage
District, and the discharge of 77,330 GPD of collected wastewater into the Bay River MSD
Wastewater Treatment Facility, pursuant to the application received on September 27, 2001, and
in conformity with the project plan, specifications, and other supporting data subsequently filed
and approved by the Department of Environment and Natural Resources, and considered a part of
this pen -nit.
This Permit shall be effective from the date of issuance until rescinded, and shall be
subject to the following conditions and limitations:
This permit shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans and specifications, and other supporting
data_

2. This permit is effective only with respect to the nature and volume of wastes described in
the application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times.
The Permittee shall maintain compliance with an individual system -wide collection system
permit for the operation and maintenance of these facilities as required by
15A NCAC 2H .0227. If an individual permit is not required, the following performance
criteria shall be met as provided in 15A NCAC 2H.0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to land or surface waters, and any contravention of the groundwater
standards in 15A NCAC 2L.0200, or the surface water standards in
15A NCAC 2B.0200.
b. A map of the sewer system shall be developed prior to January 1, 2004, and shall be
actively maintained.
C. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every
day (i.e. 365 days a year). Pump stations that are connected to a telemetry system
sball be inspected at least once per week
e. High-priority sewer lines shall be inspected at least once per every six-month period
of time.
£ A general observation of the entire sewer system shall be conducted at least once per
year.
g. Inspection and maintenance records shall be maintained for a period of at least three
years.
h. Overflows and bypasses shall be reported to the appropriate Division regional office
in accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as
required by North Carolina General Statute 143-215.1C.
4. The sewage and wastewater collected by this system shall be treated in the Bay River MSD
Wastewater Treatment Facility prior to being discharged into the receiving stream.
5. This permit is not transferable_ In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be
submitted to the Division of Water Quality (Division) accompanied by an application fee,
documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of this request will be considered on its merits, and may or may
not be approved.

6. Construction of the sewers, pump station(s), and force main shall be scheduled so as not to
interrupt service by the existing utilities, nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.
7. Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting materials. If this project is to be completed in phases
and partially certified, you shall retain the responsibility to track further construction
approved under the same permit, and shall provide a final certificate of completion once
the entire project has been completed. Mail the Certification to the Construction Grants
and Loans Section, 1633 Mail Service Center, Raleigh, North Carolina 27699-1633.
(Engineer's Certification Form is attached.)
8. A copy of the approved plans and specifications shall be maintained on file by the
P ermittee for the life of the project.
9. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division, in accordance with North Carolina
General Statutes 143-215.6A to 143-215.6C.
10. The issuance of this permit does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
11. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those
as may be required by this Division, such as the construction of additional or replacement
wastewater collection facilities.
12. The facilities shall be constructed in accordance with the requirements of the North
Carolina Sedimentation Pollution Control .Act.
13. Any abandoning of the existing facilities must be done in accordance with the
requirements of the Division of Water Quality's Washington Regional Office.
NONCOMPLIANCE NOTIFICATION:
14. The Permittee shall report by telephone to the Washington Regional Office, telephone
number [phone], as soon as possible, but in no case more than 24 hours, or on the

next worldng day following the occurrence or first knowledge of the occurrence of either
of the following:
a- Any process unit failure, due to known or unknown reasons, that renders the facility
incapable of adequate wastewater transport, such as mechanical or electrical failures
of pumps, line blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to
receiving waters without treatment of all, or any portion of the influent to such
station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter
form within five (5) days following first knowledge of the occurrence. This report must
outline the actions taken or proposed to be taken to ensure that the problem does not recur.

the State of North Carolina, having been authorized to observe (periodically/weekly/full time) the
construction of the Reelsboro Collection System serving the Western Bay River Metropolitan
Severer District area, located in Pamlico County, hereby state that to the best of my abilities, due
care and diligence was used in the observation of the following construction:
Approximately 25,375 linear feet of 8 -inch, 18,520 linear feet of 6 -inch, 18,565 linear feet of
4 -inch, 17,300 linear feet of 3 -inch and 7,600 linear feet of 2 -inch pressure sewer main;
approximately 31,145 linear feet of 1 -inch, 3,495 linear feet of 1.25 -inch, and 2,150 linear feet of
1.5 -inch of pressure service line; directional bore with 12 -inch pipe for Goose Creep crossing;
directional bore with 4 -inch pipe for Moore's Swamp crossing, West Goose Creek crossing and
Bennett Farm Road crossing; construction of 250 S.T.E.P. systems; associated site work, and
appurtenances to serve the Western Bay River Metropolitan Sewerage District, pursuant to the
application received on September 27, 2001, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources.
I certify that the construction of the above -referenced project was observed to be built within
substantial compliance and intent of the approved plans and specifications.

The Bay River Metropolitan Sewer District currently owns and operates all major wastewater
treatment facilities located within Pamlico County including the Bayboro and Oriental
Wastewater Treatment Facilities and the Arapahoe Spray Irrigation Site operating under a non -
discharge permit (Permit No. WQ0013348).
The proposed Western Bay River Metropolitan Sewerage District (WBRMSD) Service Area
Extension (304 users) is projected to generate 60,800 GPD. This generated flow would be
conveyed through the new and existing pressure sewer collection system to the Bayboro WWTF
for treatment prior to discharge at the Arapahoe Spray Site Facility. The WBRMSD flow would
increase the annual average flow to the Bayboro WWTF from the current ADF of 0.185 MGD to
0.246 MGD, which is well within the rated design capacity of 0.3 MGD. The flow to the
Arapahoe Spray Site would increase from the current ADF of 0.331 MGD to 0.392 MGD, which
is also well within the 0.5 MGD design capacity of the site.
Bay River Metropolitan Sewerage District has adequate reserve capacity to accommodate the
projected WBRMSD sewage flow and has made commitments to accept sewage flows upon
project completion.

This letter is to notify you, as an adjacent riparian landowner of Goose Creek crossing NC 55
approximately 500 feet west of the SR 1105, Prescott Road intersection near Reelsboro, North
Carolina, that Pamlico County plans to construct an 8" sewer line in the N.C. Department of
Transportation right-of-way crossing under said Goose Creek. The attached sketch accurately
depicts the proposed construction. The required landowner notification process for the CAMA
permit was previously completed on February 9, 2000 but was expired due to funding delays.
The project funding was recently approved and we are now obligated to obtain a CAMA permit.
Should you have no objections to this proposal, please check the statement below, sign and
