Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythat this project is consistent with the North Carolina Coastal Management Program.

the specified date(s) for the following. demolition of damaged vessel.
Lee's Landing Upper Broad Creek August 29 - 31, 2006
This permit is subject to the following conditions-
1 . All activities associated with this use shall be performed in a manner that does not impede
the intended use as a public boating access area.
2. Vessel demolition and removal from the site shall be completed by 5:00 pm, August 31,
2006.
3. Special care shall be taken when dealing with materials and fluids that may cause
environmental contamination. All materials and fluids including but not limited to diesel
fuel, hydraulic fluid, battery acid, and antifreeze shall remain confined and be disposed of in
an approved and environmentally sensitive manner.
4. The vessel owner will be responsible for clean up and containment in the event materials or
fluids leak from the vessel. Additionally, the owner will have on site during the demolition
spill containment materials and the contact information for an emergency spill containment
unit.
5. All litter or debris associated with this use will be picked up and removed from the area
immediately following use of the site.
6. Any damages to the site resulting from this operation shall be fully repaired within 15 days of
the occurrence.

instructions from NC Wildlife personneL
FAILURE TO ABIDE BY THE CONDITION5 WILL RESULT IN REVOCATION OF THIS
PERMIT.

the access area for this permitted use.
INDEMNIFICATION
The permiee assumes all liability and agrees to defend, indemnify, and hold harmless the North
Carolina Wildlife Resources Commission except for the conduct of the North Carolina Wildlife
Resources Commission or any of its employees in the performance of the permitted event.
