This permit is subject to compliance with this application, site drawing (\
and attached general and specific conditions. Any violation of these terms applicant's signature
may subject the permittee to a fine, imprisonment or civil action; and
may cause the permit to become null and void.
This permit must be on the project site and accessible to the permit of- permit officer's signature
ficer when the project is inspected for compliance. The applicant certi-
fies by signing this permit that 1) this project is consistent with the local —
land use plan and all local ordinances, and 2) a written statement has
been obtained from adjacent riparian landowners certifying that they _
have no objections to the proposed work.

Dear Mr. Johnson:
Attached is General Permits #C-28194 & 28195 to replace existing bridge with 25' box culvert
and replace existing culvert with a larger culvert in Pamlico County at SR 1125 Moore's Swamp
Road Bridge # 5 'h mile north of Hwy 55 in Reelsboro. Also, in Beaufort County at SR 1904
adjacent to an unnamed tributary to Cambell Creek '/z mile south of 33 near the town of Cambell
Creek at Cypress Swamp Road.
In order to validate this permit, please sign all three (3) copies as indicated. Retain the white
copy for your files and return the signed yellow and pink copies to us in the enclosed, self-
addressed envelope.

plans to replace an existing pipe with a 60' long 103" x 7 1 " aluminum arch pipe onsite of his/her property located on SR 1904
in Beaufort County. The sketch attached accurately depicts the proposed construction.
Written comments must be received by the Division of Coastal Management within 10 days of receipt of this notice.
NO RESPONSE WITHIN 10 DAYS of receipt of the correspondence will be INTERPRETED AS NO OBJECTION.
