Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

Attached is General Permit #C-30556 to install riprap in erosion pockets 5' past NWL in Onslow
County between Webb and Freeman Creek at SR1442 (Stella Road) on the Causeway west of the
bridge in Stella.
In order to validate this permit, please sign all three (3) copies as indicated. Retain the white
copy for your files and return the signed yellow and pink copies to us in the enclosed, self-
addressed envelope.

SUBJECT: Reapplication for Shoulder repair on SR 1442, Onslow County
Dear Mr. Arrington:
Please find enclosed a copy of the CAMA General Permit and application for SR 1442 (Stella Rd,) in
Onslow County. The North Carolina Department of Transportation was unable to initiate construction of
this project before the existing CAMA permit expired. The Department still proposes to repair the two
areas of erosion on the causeway shoulder on the west side of the Stella Bridge adjacent to the White Oak
River in the same manner (backfill and riprap) as described in the attached permit_ At the present, less than
two feet of shoulder exist at these areas which creates a safety hazard.
The Army Corp of Engineers, Wilmington District Office has issued a NWP 13 for this project. If you
