Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that 1) prior to undertaking any activities authorized by this permit, the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
Iandowner(s) .
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief, certifythatthis project is consistentwith the North Carolina Coastal Management Program.

Dear Ms Wheeler
I respectfully request to receive from the NC DENR — Division of Coastal Management for a General Permit for
the NC Wildlife Resources Commission's Boat Access Area to the Neuse River in town of Bridgeton for a dock
repair.
On our site, previous to the pass Hurricanes, we had a 4' x 40' wooden dock on pilings, that extended into the
Neuse River at low tide conditions 25'. Because of the severe damage that was done to the dock and for the
safety of the public, we removed the damaged portions. This has left us with a dock that is 4' x 20' that extends
into the river at low tide conditions 5'. We would now like to receive a General Permit to repair the dock back to
original size of 4' x 40' wooden dock on pilings. All work will be done in the foot print of the original dock.
If there is anything I can do to expedite the review and issuing of the permit please let me know.

General Statute 113-229 requires that riparian landowners with property adjoining a proposed
excavation and/or fill project be given thirty (30) days in which to comment on the project. This form
allows the adjacent riparian landowner to express either: (1) objection to the project or, (2) no
objection to the project and the desire to waive the right to the 30-day period so that application
processing can progress more rapidly. Of course, the adjacent riparian landowner may also choose
to not sign the form at all.
