Enclosed are three (3) copies of application for stormwater outfall maintenance general permit. Per our
telephone conversations, I will send a check for fifty dollars ($50.00) when processed. Please proceed with
the permit approval. This permit is for cleaning-J--storrnwater drainage pi utfall which was the point of
public attention this past week.
Also enclosed is a map from previous application dated August 14, 1984 which shows a plan of the original
construction of the pipe headwall and outfall. I hope this can be of some use to you in your review process,
