This permit is subject to compliance with this application, site drawing
and attached general and specific conditions. Any violation of these terms
may subject the permittee to a fine, imprisonment or civil action; and
may cause the permit to become null and void.
This permit must be on the project site and accessible to the permit of-
ficer when the project is inspected for compliance. The applicant certi-
fies by signing this permit that 1) this project is consistent with the local
land use plan and all local ordinances, and 2) a written statement has
been obtained from adjacent riparian landowners certifying that they
have no objections to the proposed work.
In issuing this permit the State of North Carolina certifies that this project
is consistent with the North Carolina Coastal Management Program.

Attached is General Permit #C-26911 to construct a 124' X 4' pier with a 12' X 16' "L" at
4006 Leslie Lane, in Emerald Isle, Carteret County.
In order to validate this permit, please sign all three (3) copies as indicated. Retain the white
copy for your files and return the yellow and pink signed copies to us in the enclosed, self-
addressed envelope.

Dear Mr. Shaver,
This letter is a request for a permit to replace the pier located at 4006 Leslie Lane, Emerald Isle, North
Carolina. The original pier «ras severely damaged by Hurricane Floyd. In accordance with your
instructions , signed statements have been obtained from the adjacent property owners indicating that they
have no objection to the proposed replacement pier. Copies of these statements are enclosed , as well as a
copy of CAMA and Dredge and Fill General Pennit No. 26166, and a copy of Receipt No. 4686, Town of
Emerald Isle.
A check in the amount of $ 100.00 is also enclosed to cover the cost of the CAMA permit. Anything you
can do to expedite this request would be greatly appreciated.
