Release discovered at 1:45 PM on 02/08/14. Amount reported released was 50 lbs. No injuries or evacuations reported.DuPont Company - Fayetteville Works is located on NC Highway 87 in Bladen County. From FRO, take Highway 87 south. The entrance to the facility is approximately 18 miles from FRO on the left side of Highway 87 just before the Cumberland/Blades Comity line. DuPont Company—Fayetteville Works Complaint Investigation Report Sulfur Trioxide Release 02/08/14 Page 2 of 2 Summary of investigation. 02/11/14— Greg Reeves spoke with Mike Johnson at DuPont. Mr. Johnson indicated that the leak appeared to be from a small seep of sulfur trioxide from a valve packing gland. Mr. Johnson indicated that he had been informed by plant staff that the released amount was only 3 lbs. Mr. Johnson will call back with more information after he has checked with plant staff regarding the leak. No injuries occurred as a result of the release, and no evacuations occurred. 02/11/14 —Mike Johnson sent an email to Greg Reeves (attached) which indicated the release to be approximately 3 lbs of sulfur trioxide vapor. Since the amount released was less than the reportable quantity, and no injuries or evacuations occurred, the investigation was considered complete as of 02/11/14. \gwr Reeves, Gregory W From: Michael.E.Johnson@dupont.com Sent: Tuesday, February 11, 2014 8:55 AM To: Reeves, Gregory W Subject: DuPont S03 Release Greg: Here is the incident communication that was sent out regarding the S03 release: Technician was in the process of valving S03 feed into the vaporizer for start up of RSU. Technician noticed a small amount of smoke (vapor) coming from a manual valve in the feed to the vaporizer. S03 pump was shutdown and feed line was isolated. Currently we are working on decontamination of the line and plan for removal and replacement of valve. Precautionary reporting was made to off site agencies. Less than three pounds of S03 is known released at this time. Here is an email that the first line supervisor sent me regarding the release: Mike, On 2/8/14 we had a small S03 vapor leak on a manual valve in the feed line to the S03 vaporizer. Technician in field observed a small amount of smoke coming from valve packing. No liquid leak was observed. Precautionary reporting to outside agencies was completed since the volume of the tank was greater than the RQ of S03. The way the technician noticed a leak was when he went to valve the feed into the vaporizer he noticed a small amount of smoke coming from the valve packing. He immediately stopped the S03 pumps and isolated the leak. Upon further investigation using the S03 tank weight from IP21, it showed a weight drop that concerned both James Daniels and I so we decided to complete the precautionary off-site reporting. After looking at the weight in the tank on the morning of 2/9/14, the weight drop appears to be weigh cell fluctuations. Therefore, the precautionary notification made to comply with the CERCLA/EPCRA "immediate" reporting requirement stated 50 lb. as the released quantity versus the regulatory reportable quantity of 100 lb, for sulfur trioxide. The actual release has been determined to have been less than 3 lb. Hope this answers your questions. Mike Michael E.
