Scott Graboski has spoken with administration personnel at your facility pertaining to this matter on March 21,2019 and we have not yet received the annual report. This letter serves to notify you of the reporting requirements. Please be advised that any future violation can result in possible civil penalties as per North Carolina General Statute 143-215.114A. North Carolina General Statute (a) (1) and (2)provides that a civil penalty may be assessed pursuant to North Carolina General Statute 143-215.107. To assure that the report is received by the required deadline, this office suggests the submittal of an electronic copy, followed by a hard copy, to meet the compliance deadline.
