Introduction
On April 1,2020,Mr.Robert Barker,Environmental Engineer of the DAQ-WSRO,conducted a compliance inspection of
Maymead Materials,Inc. -Laurel Springs Plant. This is a greenfield site for a drum-mix asphalt plant(150 tons per hour
maximum rated capacity)and has had a permit since December 22,2005. Mr. Sean Mackey, Senior Project Manager,
was called and verified that there was still no asphalt plant located'at this site. The facility,was.last inspected on July 11,
2019 by Mr. Shannon Leonard(formerly of NCDAQ)and was found to be in compliance.
Safety
When the facility is placed into service,the only protective equipment required will be safety glasses,ear protection,
safety shoes,hard hat,and a reflector vest.
Stack Tests
There has not been stack testing conducted at this facility.
Applicable Regulations
Applicable regulations listed in the permit are 2D .0202,2D .0506,2D .0516,2D .0521,2D .0524(40 CFR Part 60,
Subpart I),2D .0535,2D .0540,2D .0605,2D 0611,2D .1100,2D .1806,2Q .0304,2Q .0309,2Q .0315,2Q .0317
(Avoidance)and 2Q .0711. The facility will not be subject to RMP requirements of the 112(r)program since it will not
use or store any of the regulated chemicals in quantities above the threshold levels in the rule. It should be noted that the
facility will be subject to the General Duty clause of the 112(r)program.
Discussion
The facility is permitted for a No. 2/recycled No.4 fuel oil-fired aggregate dryer(ES-1, a hot mix asphalt storage silo(ES-
2),and a truck loadout operation(ES-3). The facility is also permitted for four(4)insignificant sources consisting of two
(2)fuel oil storage tanks(IES-1 an IES-2), a liquid asphalt cement storage tank(IES-3), and a no. 2 fuel oil-fired asphalt
storage tank heater(IES-4). As stated earlier,none of these sources have been constructed at the facility site.

Upon start-up,the facility is required to submit quarterly and annual reports. The quarterly report is for the amount of
asphalt produced,fuel certification records, and sulfur dioxide emissions,while the annual report is for the results of
analytical testing of the fuel and amount used. Even though the facility is not yet constructed,the facility is still
submitting these reports. The last quarterly and annual reports were last received on January 3,2020. The reports stated
that there was no functioning facility at the location.
