Dear Ms. Carter,
Per your request,below is a summary of the November 22,2019 incident at the Fayetteville Works
facility.'
On November 22, 2019, a release of Perfluoropropionyl Fluoride (PPF) occurred in the Vinyl
Ethers North tower. Stack testing was occurring in the area of the VEN tower, and the release
(unrelated to the testing) was identified by personnel participating in the testing and site
technicians. Upon investigation, Chemours determined that an overhead line from the Acid
Fluoride (AF) Stripper Column to the condenser had a pinhole leak in the pipe. Chemours
technicians followed shutdown and decontamination procedures to allow for the needed repairs.
Personnel potentially exposed to the chemical received precautionary treatment via nebulizer.
During treatment and observance period, none of the personnel exposed showed any signs or
symptoms.
Corrective actions for this incident were:
• Include the use of ELSA escape packs in contractor safety orientation - complete
• Setup Preventative Maintenance to inspect AF Column overhead piping when column is
repacked—set up in SAP
