Dear Mr. Himebaugh:
Enclosed is the Final Agency Decision of the Environmental Management Commission
assessing civil penalty and investigative costs against Piedmont Composites and Tooling, LLC
totaling$2,302.00. According to North Carolina General Statutes § 143-215.5 and § 150B-45,
you may seek judicial review of the Commission's decision in the appropriate Superior Court by
filing a petition within 30 days of receipt of the written copy of the Final Agency Decision. A
copy of the judicial review petition must be served on the Commission's agent for service of
process at the following address:

If you choose to file a petition for judicial review, I request that you also serve a copy of

mailing a check made payable to the Department of Environmental Quality to
