Specific Condition and Limitation Nos. 6.d.ii of said permit states in part: Reporting Requirements — In addition to any other notification requirements to the Environmental Protection Agency (EPA), the Permittee is required to NOTIFY the Regional Supervisor, DAQ, in WRITING, of the following: ii. The actual date of initial start-up of an affected source, postmarked within 15 days after such date. The report due on November 12, 2020 was received by this office on March 11, 2021. Therefore, this Notice of Deficiency is issued to Substance Incorporated for failure to submit the required report within the required timeframe in violation of Air Permit No. 10649R00, Specific Condition and Limitation No. 6.d.ii. Your response should describe any corrective actions that have been taken by the company to assure compliance with the referenced permit condition at all times. It should be noted that additional documented deficiencies could result in a Notice of Violation by this office. We appreciate your immediate attention to this environmental concern.
