Dear Mr. Yarbrough,
This is to acknowledge receipt at the Winston-Salem Regional Office,on February 12,2021 of
the Protocol Submittal Form for the filterable particulate matter(PM),multiple metals(Pb,Cd,&Hg),
hydrochloric acid(HCI),and visible emissions tests scheduled for the week of April 5,2021 at the
above referenced facility. The emission sources to be tested are the two dual-chamber,natural gas-
fired hospital,medical,and infectious waste incinerators (Emission Source IDS: ES-01 and ES-02).A
copy of this Protocol Submittal Form will be forwarded to our Stationary Source Compliance Branch
in Raleigh for review. They will notify you of the results of that review when it is completed.
The instructions for the Protocol Submittal Form state that the form should be submitted at
least 45 days prior to testing to allow the Division sufficient time for review. Your submittal did meet
this requirement for the tests scheduled the week of April 5,2021. Should you conduct the test without
receiving approval from the Stationary Source Compliance Branch,the test may not be accepted. If
