Directions:
From MRO,travel Hwy. 150 west toward Lincolnton. Just past the Hwy. 16 intersection,turn right onto
E. Maiden Road. Travel approximately 12 miles to Maiden. Go through town on Hwy. 321 Business to
the stoplight where Hwy. 321 Business turns right. Go straight,travel approximately 3 miles and the
facility is located on the left.
Safety Equipment:
Hardhat, safety glasses and safety shoes will be required. The facility also requires all vehicles to be
chocked. MRO chocks are located in a box beside the storage cabinets and the facility also has
permanent concrete chocks on site.

No safety issues were noted.

A review of the"Facilities Regulated by Air Quality"on the DAQ website indicated the facility's location
is accurate and matches the facility's latitude and longitude coordinates listed in IBEAM.No changes to
the latitude and longitude coordinates of this facility are needed in IBEAM.

1. The purpose of this site visit was to conduct a routine air quality inspection. This quarry crushes
stone using portable equipment that is only onsite for a short period of time to produce enough
stone for retail sales. The equipment was in Maiden from September of 2019 through mid-May of
2020. Mr.Nivens and I spoke on the phone on February 2,2020 and he stated that there are no
plans to have the portable equipment back in Maiden in 2021. I also drove by the facility on
February 5,2021 to verify that no equipment was on-site.

During the inspection, I verified the facility contact information in IBEAM.No changes were
needed.

3. COVID-19Information:
Currently visitors are not encouraged at the facility. However,if an inspection is required masks
would be required and the inspector's temperature would be taken and then allowed onto the
property with no fever. Employees are required to wear face masks. Before my drive-by on
February 5,2021,I filled out the NC DEQ"NC Employee Screening for Employees Reporting
Onsite"questionnaire on my cell phone.
4. Compliance history file review:
No problems have been noted in the last five years by DAQ,prior to this inspection. The current
compliance status is discussed in the following sections.
5. Observations of permitted air emission sources and control devices:
I drove by the quarry on February 5,2021 to verify that no equipment was on-site. I did not find
any equipment located at the facility.
6. Observations of insignificant air emission sources and control devices listed on the current
>l ennit:
None noted.
7. Observations of air emission sources and control devices not listed on the current permit:
None noted.
8. Compliance with specific permit conditions and limitations:
a. Condition A.2.—"Emissions Inventory Requirement". At least 90 days prior to the
expiration date of the permit,the Permittee shall submit the air pollution emission
inventory report. The report shall be submitted to the Regional Supervisor,DAQ. The
report shall document air pollutants emitted for the 2020 calendar year.
Observed: The Air Permit will be expiring on September 30,2021. I reminded Mr.
Nivens that the permit renewal application must be submitted at least 90 days prior to
the permit expiring. The permit renewal application must also include an emission
inventory for calendar year 2020. 1 instructed Mr.Nivens to contact DAQ-MRO if he
has any questions or concerns regarding the permit renewal.
b. Condition A.6.— 15A NCAC 2D .0524 NSPS Subpart 000
Observed: As required by NSPS subpart 000,the facility is required to conduct
monthly spray nozzle inspections.Mr.Nivens supplied me(via email)with the monthly
spray nozzle inspections performed by the facility while the equipment was onsite
between September of 2019 and May of 2020.

9. NSPS/NESHAP Review:
No boilers, generators, or fire pumps are located at the facility. Mr.Nivens also stated that the
facility did not have any gasoline storage tanks located on the property.
10. Summary of changes needed to the current permit:
None.
11. Compliance assistance offered duringthe h�pection:
None.
12. Section 112(r)applicability:
This facility does not appear to be subject to the requirements of the Chemical Accident Release
Prevention Program, Section 112(r)of the Clean Air Act.
13. Compliance determination:
Based on my observations,this facility appeared to be in compliance with the applicable air
quality regulations at the time of the inspection
