Travel about 1 mile West on NC 10 and then turn left onto Johnson Bridge Road. Travel about 2 miles South on Johnson Bridge Road and then turn right on a paved road just before crossing the river(look for sign at the entrance to the facility). Facility is located at the end of this road. Safety Equipment:Hardhat, safety glass, safety shoes, earplugs. 1. Facility Overview Discuss ion/Introduction On March 11,20211 arrived at the site to verify that the facility is not in operation. The gate at the entrance was padlocked and it did not appear that there had been any recent traffic on the driveway. According to letters, annual reports,and recent inspection reports,the facility has not operated since the permit was issued to Maymead in December 2010. Facility Contacts The facility contacts are up to date in IBEAM. No one was on site to escort me for an inspection. Facility Description The facility is a hot mix asphalt batch plant rated at 100 tons per hour maximum capacity. 2. Compliance with specific permit conditions and limitations Compliance with the permit conditions will be verified if the plant begins operating(visible emissions,I&M logs, production,fuel,etc.).The facility stated in a July 2010 letter that the plant will not operate in the foreseeable future (ref: recent telephone discussion with Mr. Sean Mackey). Specific Condition 10 of the permit requires a notification within 15 days of the start-up and Specific Condition 8 requires Methods 5 and 202 stack testing within 60 days of the start-up. 3. Compliance History Review The facility was issued a Notice of Deficiency on February 10,2017 for a late annual report. There have been no other documented violations by the facility in the past five years. 4. Stack Test Review Specific Conditions 8 of the permit requires Methods 5 and 202 stack tests within 60 days of start-up to demonstrate compliance with NSPS, Subpart I and 2D .0506. Based on the last tests conducted,the facility is expected to be capable of passing the required stack tests. The results of the September 2005 stack test are shown below.
