Dear Mr. Hildreth,
On 16 June 2021, Mike Turner, with the Fayetteville Regional Office of the Division of
Air Quality (FRO DAQ), met with Office Manager, Chanel Little, to conduct an air quality
compliance inspection at Hildreth Ready Mix, LLC in Wadesboro, Anson County, NC. During
the inspection, Mr. Turner noted the documentation for bagfilter inspections did not describe the
inspection and maintenance performed, any variances from requirements, and corrections of
those variances. Additionally, Ms. Little was unable to demonstrate that a copy of your current
air permit is kept on site.
Your current permit contains the Specific Condition and Limitation A.7, 15A NCAC 02D
.0611 — Fabric Filter Requirements, which requires that "...all inspections and any variance
from manufacturer's recommendations or from those given in this permit (when applicable),
shall be investigated with corrections made and dates of actions recorded in a logbook. Records
of all maintenance activities shall be recorded in the logbook. The logbook (in written or
electronic form) shall be kept on-site and made available to DAQ personnel upon request."
Failure to maintain records of the results of the inspections, any corrective actions, and
dates of those actions is a deviation from the requirements of 15A NCAC 02D .0611
"Fabric Filter Requirements."
Your current permit also contains the General Condition and Limitation B.14 15A NCAC
02Q .0110 — Permit Retention Requirement, which states that "...the Permittee shall retain a
current copy of the air permit at the site. The Permittee must make available to personnel of the
DAQ, upon request, the current copy of the air permit for the site." Failure to retain a current
copy of the facility's air permit at the site is a deviation from the requirements of 15A
NCAC 02Q .0110 "Permit Retention Requirement."

Please note that inspectors from this office have repeatedly advised you of these permit
requirements regarding recordkeeping of bagfilter inspection and maintenance activities and on-
site permit retention. Be advised, you were issued a Notice of Deficiency in 2020 for failing to
maintain adequate records of bagfilter inspection and maintenance.
As stated in the "subject" above, this letter represents a Notice of Violation for the above-
cited violations. These violations and any future violation of an air quality regulation is subject
to the assessment of civil penalties per North Carolina General Statute 143-215.114A. In
addition, each day of continuing violation after written notification from the Division of Air
Quality shall be considered a separate offense.
We are concerned about what appears to be a pattern of inadequate recordkeeping for this
facility. Please submit a written response to this office by 14 July 2021. In your response,
please detail what steps you have taken, or plan to take, to ensure compliance with the cited
requirements and the date by which you anticipate achieving compliance.
We recommend that you carefully examine the requirements in your current air permit.
