Dear Mr.Barrett:
On November 3,2021,Ryan Dyson of this office contacted you and conducted a compliance inspection of
the above referenced facility. During the course of the inspection,Mr.Dyson discovered that a track-mounted
portable crusher with a built-in conveyor was placed in operation at the facility in March of 2021. This portable unit
was not included on the facility's equipment list,which was dated June 28,2017. By failing to maintain a current
equipment list on-site,you are deficient in fulfilling the requirements of 15A North Carolina Administrative Code
(NCAC)2D.0501(c),as referenced by Condition A.3 of Air Quality Permit No.07832R04,which states in part,
"The Permittee shall maintain on-site an equipment list and a plant(or flow)diagram of all equipment covered
under this permit."By failing to notify the Regional Supervisor,DAQ,in writing,of the actual date of initial start-up
of this portable unit postmarked within 15 days after such date,you are also deficient in meeting the requirements of
40 CFR Part 60, Subpart 000,as referenced by Condition A.8 of Air Quality Permit No. 07832R04.
Please advise this office in writing no later than November 24,2021,of the cause of this deficiency,what
was done to rectify the situation,and what will be done to ensure that this type of deficiency does not occur in the
future. Additionally,this office recommends that you or a designated company representative review your air
permit to ensure that you understand all the specific and general permit conditions.
Failure to fulfill the requirements of permit conditions A.3 and A.8 in the future may result in a violation.
All air quality violations are subject to enforcement action,including a civil penalty pursuant to North Carolina
General Statute(NCGS) 143-215.114A or permit revocation as authorized by NCGS 143-215.108(b)(3). You may
