Air Permit No. 03981R15
Dear Mr. Kreidler:
Air Permit No. 03981R15 was issued to Bassett Upholstery Division, Newton, Catawba County,
North Carolina on August 10, 2016, with an effective date of August 10, 2016. Specific Condition and
Limitation No. A.15.c. of said permit states in part:
"i. The Permittee shall record monthly and total annually the following:
A. The type and arnouizt of fuel combusted in the boilers (ID Nos. ES-B-2 and ES-B-3)
B. The facility-wide sulfur dioxide, nitrogen oxides, carbon monoxide, VOC, highest
individual HAP, and total HAP emissions.
ii. Fuel supplier certification shall be kept on-site and made available to DAQ personnel upon
request.
iii. A log book shall be kept on site for each control device and made available to Division of Air
Quality personnel upon request. The Permittee shall record all inspection, maintenance and monitoring
requirements listed above in the log book. Any variance from the manufacturer's recommendations shall
be investigated with corrections made and date of actions recorded in the log book."

Mr. Joe Foutz of this office performed an inspection of your facility on August 23, 2022. During
that inspection, monthly records were not available for any month in, calendar year 2022. Therefore, a
Notice of Deficiency is issued to Bassett Upholstery Division for _a deviation from Air Permit No.
03981R15 Specific Condition and Limitation No. A.15.c.
Also, Specific Condition and Limitation No. A.14.b.ii.A. of said permit also states in part:
"For the boiler(ID No. ES-B-2) and-boiler(ID No. ES-B-3),.a biennial tune-up is required and
shall be conducted no more than 25 months after the previous tune-up. The following units are
only required to conduct a tune-up every five years: seasonal boilers (shutdown for 7 consecutive
months or 210 consecutive days each 12-month period due to seasonal conditions; only biomass
or oil), limited-use boilers, and units with oxygen trim systems, as defined in 40 CFR 63.11237.
Each five-year tune-up must be conducted within 61 months of the previous tune-up."
During the August 23, 2022, inspection, no records were,available regarding maintenance on the
boilers. However,it was later determined the most recent tune-up for each boiler was conducted April 17,
2019. The boilers do not qualify as seasonal or limited use boilers .because records indicate wood was
combusted in the boiler every month in calendar year 2021. As a result, a tune-up has not been performed on
the boilers within the required timeframe of at least 25 months from the previous tune-up. Therefore, a
Notice of Violation is issued to Bassett Upholstery Division for violation of Air Permit No. 03981R15
Specific Condition and Limitation No. A.14.b.ii.A.
Furthermore, General Condition and Limitation No. B.6 of said permit also states in part:
"In accordance with G.S. 143-215.108(c)(1),the facility shall be properly operated and maintained
at all times in a manner that will effect an overall reduction in air pollution."
During the August 23, 2022, inspection,Mr. Foutz observed visible emissions/wood dust escaping
and venting from a trailer used to store and haul wood waste. Therefore, a Notice of Violation is issued to
Bassett Upholstery Division for violation of Air Permit No. 03981RI5 General Condition and Limitation
No. B.6
It is requested that a written response to this Notice of Deficiency and Notice of Violation be
submitted to this office as soon as possible,but no later than ten days following receipt of this notice.
Your response should describe any corrective actions that have been taken by the company to assure
compliance with the referenced permit conditions at all times.
It should be noted that additional documented deficiencies could result in a Notice of Violation by
this office. In addition, the violations cited in this letter or any additional documented-violations could
result in the initiation of an enforcement action by this office in accordance with North Carolina General
Statute 143-215.114A, which provides for the assessment of civil penalties.
