Dear Mr. Cottrell:
On January 17,2013,Betsy Huddleston of this office conducted a compliance inspection of the
subject facility. During her inspection file review,Ms.Huddleston discovered that the Site Specific
Monitoring Plan and the Notice of Compliance Status(NOCS)required by 40 CFR Part 63, Subpart
JJJJJJ had not been received by this office. The Site Specific Monitoring plan should have been
submitted at least 60 days prior to the performance testing on May 10,2012(per 40CFR 63.63.11205(c)
and permit specific condition A.13.d.iii.). The NOCS should have been submitted within 60 days
following the May 10, 2012 compliance test(per 40CFR 63.9(h), 63.11225(a)(4)and permit specific
condition A.131iii.). Both the Site Specific Monitoring Plan and the NOCS were received by this office
on February 13,2013.
Please note the Boiler GACT monitoring,recordkeeping and reporting requirements in Permit
10126RO1, and make the necessary provisions to ensure that these requirements are met within the
specified time frames. In the future,failure to submit the required reports will be considered a
violation of your air permit. Any violation of air quality regulations is subject to the assessment of civil
penalties as per North Carolina General Statute 143-215.114A;
