Bakelite Chemicals LLC, located in Conway, NC is required to submit quarterly deviation reports for those
deviations not covered under I SA NCAC 2D.0535 in accordance with Section 3: General Conditions, J.A.3.a of
the faci lity's Title V pcnnit. As such, GP has enclosed duplicate copies of the referenced quarterly deviation
report intended to satisfy the above quarterly reporting requirements.
