( ) YES, the subject facility is in compliance with all relevant standards and other requirements of 40 CFR 63 DDDDDDD. Since there were no deviations during the reporting year, this certification has been completed (by March 1st of the following year) and filed at the facility for on-site review by the permitting agency. (X ) NO, the subject facility did deviate from the relevant standards of 40 CFR 63 DDDDDDD. The attached document describes the deviation(s), the time period of the deviation, and corrective actions taken. This information will be filed on site at the facility as well as submitted by March 1st to the permitting agency. Signature of Responsible Official: Date: -aa-ZO 8 p Murphy-Brown, LLC dba/Smithfield Hog Production 2-28-2020 Notice of Murphy-Brown deviation and corrective action pursuant to 40 CFR 63 7D. During our onsite inspection on October 15th it was discovered that Murphy-Brown had been receiving Trace mineral in bulk and not in bag form. The State had not been notified of this. This was directly due to miscommunication and lack of knowledge on Murphy-Brown's behalf. Since the inspection housekeeping requirements and records have changed to accommodate the manganese in the Trace mineral. Murphy-Brown / Bladenboro Mill strives to comply with all applicable regulations and takes its role in protecting the environment seriously! Please be assured that steps have been taken to ensure this issue does not happen again. Your continued guidance is greatly appreciated as we bring management stability to the Bladenboro Facility.
