The application for renewal must be received by the Division of Air Quality as specified in General Condition 3.K of your Title V Permit. Failure to submit this application by this deadline may terminate the facility's right to operate pursuant to 15A NCAC 2Q .0513(b). In order for your renewal request to be considered complete and obtain the application shield that will allow your facility to continue to operate under the terms of your permit, a completed Form A, Form E5, and Form E6 should be filled out in their entirety and submitted to the Fayetteville Regional Office. You are required to include an evaluation of the applicability of the Compliance Assurance Monitoring (CAM) rule pursuant to 40 CFR Part 64 in your Title V permit renewal application in accordance with Section 5 of Form E6. In addition, the remainder of Form E6 must be completed for each CAM-affected Pollutant Specific Emission Unit (PSEU) identified at the facility (if applicable). A complete renewal request must include at least one original and two additional copies of the following: i a. a completed Form A"Administrative Application" including Section AA2, on page 2 "Application for Title V Permit Renewal;" b. a completed Form E5 "Title V Compliance Certification." The compliance certification should apply to the time period "year to date" (i.e., if the form is submitted on September 10, the compliance certification time period should be January 1 through September 10); c. as part of the permit renewal process, this Office will also need to know if this facility is subject to 40 CFR Part 68, Prevention of Accidental Releases, Section 112(r) of the Federal Clean Air Act. Please complete the Form A3 for this information; d. a completed Form E6 "Compliance Assurance Monitoring (CAM) Plan;" AND e. MACT review. Note that these required forms must be signed by a responsible official of the subject company pursuant to 15A NCAC 2Q .0520 and defined in 40 CFR Part 70.2 as follows: a. for corporations, a president, secretary, treasurer, or vice-president in charge of a principal business function, or any other person who performs similar policy, or a duly authorized representative of such person, if such representative is responsible for the overall operation of the subject facility and has been previously approved by the Division of Air Quality; b. for partnership or limited partnership, by a general partner; c. for a sole proprietorship, by the proprietor; d. for municipal, state, federal, or other public entity, by a principal executive officer, ranking elected official, or other duly authorized employee. There is no fee for renewal of air permits. If for any reason you do not wish to renew your permit, please inform this Office in writing. If any modifications have been made or new equipment has been added which result in any increase in emissions, you may be required to file a new permit application for this equipment. Please contact our office at 910-433-3300 to determine if you are required to submit an application for additional equipment not previously permitted. Mr. Howard Brown, Jr. July 19, 2022 Page 3 Furthermore, if you are requesting information to be held as confidential, you must submit written justification as to how the information you have marked confidential should be held as such. This request must include the following: a. How the information meets the definition of"Trade Secret" per NCGS 132-1.2 and NCGS 66-152(3); b. How the information is not "Emissions Data" as defined in 40 CFR 2.301; c. How your facility would be affected if the information is disclosed; and d. How the information is protected at your facility. Only one copy of those pages which contain confidential information is required to be submitted.
