Attachment 1 details the semi-annual requirements of the Title V permit and the facility's method of compliance. Attachment 2 consists of supporting documentation that are being submitted in accordance with the requirement of Section 2 - 2.1.A.4(m) of the Title V permit. Attachment 3 includes the statement of certification from the Responsible Official.
