I. DIRECTIONS TO FACILITY: From the Raleigh Regional Office(RRO)of the North Carolina
Division of Air Quality(DAQ), turn right onto Six Forks Road. Go to the second signal light and turn
left to take 1-440 W. Turn left to merge onto I-440. Go 7.6 miles and keep left to continue on US-I
South. Go 32.5 miles and take the exit onto US-421 North. Go 8.7 miles and turn left onto S. Main
Street(Goldston). Note: The roads in this area were under construction at the time of this inspection so
you may want to use mapping software or to contact the facility to make sure of the best route to take to
get to this site.
ll. FACILITY DESCRIPTION: This is a quarry located in Goldston, Chatham County,NC that mines,
crushes,and screens rock and gravel.
SAFETY: Standard safety items (i.e., hard hat, safety shoes,and hearing/eye protection)will be needed,
as well as a safety vest and tire chock.
111. INSPECTION SUMMARY: On December 7, 2021, I (Dena Pittman) visited the site in Goldston,North
Carolina and met with Ryan Seremet, Plant Manager,and Steve VanDeventer to conduct the compliance
inspection. We reviewed the permit and then drove around the site to view all the equipment. There was
maintenance being conducted on the crusher at the time of the inspection and most of the operations were
shut down. Mr. Seremet was able to provide a nozzle inspection form and equipment listing for the site
and sent these documents to me by email on December 13, 2021.
I explained to Mr. Seremet that the conditions in the permit would be changing since NSPS Subpart 000
and odors are not included in the current permit. In addition, I explained that some of the quarry
stipulations have been revised so there will also be changes to the current permit conditions.

Appeared to be in compliance. All permitted equipment was observed.
V. SPECIFIC PERMIT CONDITIONS:
A.I Applicable Regulations: The Permittee shall comply with 15A NCAC Subchapter 2D .0202,2D .0510, 2D
.0521, 2D .0535, 2D .0540 and 2D 0605.
Appeared to be in compliance—see below.
A.2 PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT-At least 90 days before
the permit expiration date the Permittee,shall request permit renewal and submit emission inventory report for
the 2020 calendar year.
Appeared to be in compliance. Facility submitted an application to renew their permit by the deadline.
I told Mr. Seremet that the next renewal would be due in 8 years.
A.3 PARTICULATE CONTROL REQUIREMENT - As required by 15A NCAC 2D .0510
"Particulates from Sand, Gravel, or Crushed Stone Operations," the following requirements apply:
a. The Permittee shall take measures to reduce to a minimum any particulate matter from becoming airborne
to prevent exceeding the ambient air quality standards beyond the property line for particulate matter.
b. Fugitive dust shall be controlled by 15A NCAC 2D .0540 "Particulates from Fugitive Dust Emission
Sources."
c. The Permittee of any sand,gravel, or crushed stone operation shall control process-generated emissions:
i. From crushers with wet suppression; and
ii. From conveyors, screens,and transfer points
such that the applicable opacity standards in 15A NCAC 2D .0521 Control of Visible Emissions,"
or 15A NCAC 2D .0524 "New Source Performance standards" are not exceeded
Appeared to be in compliance. Wet suppression appeared to be working adequately throughout the
plant.
AA VISIBLE EMISSIONS(VE)CONTROL REQUIREMENT-As required by 15A NCAC 2D.0521,
VE from the emission sources, manufactured after July 1, 1971, shall not be greater than (>) twenty percent
(20%)opacity when averaged over a six-minute period.
Appeared to be in compliance. There were no notable visible emissions(VE)observed.
A.5 VISIBLE EMISSIONS(VE)CONTROL REQUIREMENT-As required by 15A NCAC 2D.0521,
VE from the emission sources, manufactured before July I, 1971, shall not be greater than (>) forty percent
(40%)opacity when averaged over a six-minute period.
Appeared to be in compliance. There were no notable visible emissions (VE)observed.
A.6 NOTIFICATION REQUIREMENT - As required by 15A NCAC 2D .0535, the Permittee of a
source of excess emissions that last greater than(>)four(4)hours and results from a malfunction,a breakdown
of process or control equipment or any other abnormal conditions, shall notify DAQ by the next business day.
Appeared to be in compliance. Mr. Seremet indicated that they had not operated in an upset state.
A.7 FUGITIVE DUST CONTROL REQUIREMENT - As required by 15A NCAC 2D .0540, the
Permittce shall not cause or allow fugitive dust emissions to cause or contribute to substantive complaints or
excess visible emissions beyond the property boundary.
Appeared to be in compliance. No visible emissions were observed leaving the property. The driveway
was paved and a water truck used as needed on the haul roads.
AX QUARRY EQUIPMENT REPORTING -As required by 15A NCAC 2D.0605,the Permittee shall
maintain on-site an equipment list and a plant(or flow)diagram of all equipment covered under this permit.
a. The equipment list shall include the following information for each piece of
equipment:
i. A description of equipment including applicability of New Source
Performance Standards,and:
A. Width of belt conveyors,
B. Dimensions and configuration (e.g., triple deck ) of screens, and
C. Rated capacity(tons or tons/hr)of all nonmetallic mineral
processing equipment.
ii. A unique ID number.
iii. The date the equipment was manufactured.
iv. The dates any required performance testing was conducted and submitted
to the Regional Supervisor, DAQ.
b. The equipment list and plant(or flow)diagram shall bear the date when the
current list and diagrams were revised.
c. The Permittee shall provide documentation to the Regional Supervisor, DAQ, for
any required performance testing within seven days of a written request.
d. Notwithstanding General Conditions and Limitations titled "Equipment
Relocation" and "Reporting Requirement," the Permittee may install new and
relocate existing nonmetallic mineral processing equipment. The Permittee shall
provide written notification to the Regional Supervisor, DAQ, including a revised
equipment list and plant (or flow) diagram,each time nonmetallic mineral
processing equipment is installed or relocated at a facility. This notification shall
be submitted at least IS days before the equipment is installed or relocated at the
facility unless otherwise approved by the Director.Nonmetallic mineral
processing equipment includes all crushers,screens,conveyors and loadout bins.
Appeared to be in compliance. Mr. Semeret provided the equipment list in an email on December
13,2021.
VI. INSIGNIFICANT /EXEMPT EMISSION SOURCES: There are no exempt/insignificant
sources listed in the permit; no such sources were observed during the site inspection.
VII. CLEAN AIR ACT SECTION 112(r) REQUIREMENTS: The facility is subject to the 1 I2(r)
program general duty clause but does not maintain regulated chemicals onsite above the
threshold quantities, which would require a risk management plan.
VIII. EMISSIONS INVENTORY REVIEW: There are no emissions to compare.
IX. SOURCE TEST REVIEW: No testing listed in the database.
X. COMPLIANCE HISTORY: No history of compliance issues at this site.
XI. CONCLUSIONS/RECOMMENDATIONS: At the time of this inspection, this facility
appeared to be in compliance with the requirements of their air permit. It is recommended that
the facility be inspected again within two years.
