Directions: From Mooresville,take Highway 150 West for approximately eight miles near Terrell. The
facility is located on the right.
Safety Equipment.None at this time. -
Safety Issues: None.
Lat/Lonzr "Maps of DAQ Regulated Facilities" is no longer available, but the coordinates are correct and
locked in IBEAM.
Email Contacts: The email address for both contracts is still correct.
1. The purpose of this report was to verify that the facility is still not operating and that there were no
changes since the last inspection. According to the facility contact, Mr. Allred, the facility was
officially shutdown on May 29, 2015 and all structures and equipment was removed from the site
in February of this year(see attached email).
2. Facility Contact Information:
There have not been any changes to the contact information.
3. Compliance history file review:
This facility has not had any compliance issues since the air permit was issued.
4. Compliance Assistance Offered Duringthe he Inspection:
I reminded Mr.Allred that if they did not want to keep the permit open,that they could send in a
request for permit recension.
5. Compliance Determination:
Based on my correspondence with the facility contact,this permittee appeared to be in
compliance with the applicable air quality regulations.

That is correct. Our plant was taken down and removed from the property in February of 2019. Our last day of
operations was May 26, 2015.
