Stipulation No. A. 7. B. ii of Specific Conditions and Limitations of Air Permit No. 05061R6S,---
issued to Gulf States Paper Corporation, Conover, Catawba County,North Carolina on November 29,
2999, states in part: "Within thirty (30) days after each calendar year quarter, regardless of the actual
emissions, the Permittee shall submit to the Regional Supervisor, DAQ, a report summarizing
emissions of VOC and HAPs."
The report due on April 30, 2002 was received by this Office on May 6, 2002 and postmarked
on May 3, 2002. The failure of Gulf States Paper Corporation to submit the required report within the
required time frame is a violation of North Carolina Air Permit No. 05061R08 Stipulation No. A. 7. B.

It is requested that a written response to this Notice of Violation be submitted to this Office as
soon as possible, but no later than ten (10) days following receipt of this Notice. The response
should describe any corrective actions that have been taken by the Company to assure the submittal of
the required report(s) in a timely manner in the future.
It should be noted that the above violation or any additional documented violations could
result in the initiation of an enforcement action in accordance with North Carolina General Statute
143-215.114, which includes provisions for the assessment of civil penalties.
