Hildreth Septic Tanks,Air Permit No. 08715G02

Dear Mr. Hildreth:
Your current air permit, identified above, includes at least one (1) stipulation requiring that a report
be sent to this Office, no later than 1 March 2010. Your facility failed to comply with this requirement.
Your report was received by this Office postmarked after the stipulated due date. Once the report(s) is
reviewed, a compliance determination, based upon the content of the report(s), shall be made. If it is
determined during this review that your facility was not operating in compliance, an additional Notice of
Violation or a Notice of Violation/Notice of Recommendation for Enforcement may be issued to your
facility, depending on the type of violation and severity.
As stated in the "subject" above, this letter represents a Notice of Violation for failure to submit a
required report by the deadline specified in your air permit. This General Statute provides that civil penalties
of not more than twenty-five thousand ($25,000) may be assessed against any person who violates any
classification, standard, or limitation established pursuant to General Statute 143-215.107, Air Quality
Standards and Classifications.
The Division strongly recommends that you carefully examine your current permit stipulations and
to request explanation from this Office as necessary. If you have any questions regarding this matter, please

pollution prevention as they evaluate their compliance issues. Please refer to the enclosed Pollution
Prevention/Small Business insert for further information.
