Dear Mr. Horne,
Your Air Quality Permit cited above includes General Condition and Limitation B.3 which states, in part,
"PERMIT RENEWAL REQUIREMENT- The Permittee, at least 90 clays prior to the expiration date of this permit,
shall request permit renewal by letter in accordance with 15A NCAC 2Q .0304(d) and(f)." The completed renewal
application was due at this office by 3 January 2012. Despite repeated reminders and assistance from this office, as
of the date of this letter, the complete application has not been received.
This letter is to officially inform you that your failure to submit a timely, complete permit renewal
application was a deviation of the cited air permit stipulation. Be aware that future deviations from air quality
requirements could result in a Notice of Violation and corresponding penalties.
Please provide to this office the completed renewal application as soon as possible. Additionally, no
later than 6 February 2012, please provide a written response detailing what steps you have taken to ensure
future compliance with the cited stipulation. The Division strongly recommends that you carefully examine your
current permit stipulations. If you have any questions regarding this matter, please call Cindy Grimes,
Environmental Specialist, or Robert Hayden, Compliance Coordinator.
