1) Location
B.V. Hedrick Gravel and Sand Quarry-Lilesville is located on Ingram Mountain Road near Lilesville,NC,
Anson County.
Directions
From FRO,take Hwy 401 South to Wagram.Turn right at Old Wire Road(Hwy144)and go— 11 '/2 miles to
Laurel Hill.Turn right at Hwy 74 and go west about 3 0 miles to Lilesville.Turn right at flashing caution light
onto 5a'Street(at elementary school).Follow 5a'Street,and bear left to continue on Wall Street,and then go
/z mile until Stanback Feny Road. Tum right and go another%mile until Ingram Mountain Road,turn
_ right and go—1 more mile.There is a sign next to dirt drive that says"Cumberland Sand and Gravel"on left
hand side. Proposed quarry site is located past gate.
Safety considerations: Standard DAQ safety equipment,as well as safety vest; be alert for large equipment
moving on property when operations start up.
2) Facility and Process Description
B.V.Hedrick Gravel and Sand Company(Lilesville Plant)is a rock quarry that will produce aggregate for
cement and asphalt plants. This facility is permitted under Air Permit No. 6725R04, effective from 16
June 2009 until 31 May 2014. Cindy Grimes conducted the last compliance inspection on 20 April 2011.
This facility is(will be)a quarry which extracts mainly granite from the earth. The granite and crushed stone
products are loosened by drilling and blasting, and then are loaded by power shovel or front-end loader into
large haul trucks that transport the material to the processing operations.Primary operations include a jaw,
which is used for initial crusher reduction, as well as a series of conveyors. This crusher product is next fed
through a grizzly feeder for undersize material,and then is discharged onto a conveyor,where it goes through
more screening processes,until it is conveyed to a surge pile for temporary storage or sold as coarse
aggregate.

3) Inspection Summary
On 20 June 2013, I Mike Thomas of FRO DAQ drove to this site to verify that the facility was not
operating. There was a locked gate across the dirt driveway;there was no evidence or indication that
any activities have taken place at the facility in several years.
4) Permit Stipulations
The facility has not yet initiated operations at this time. See permit for stipulation details.
5) Reporting Requirements
Within 15 days after start-up of the rock crushing operation,the Permittee shall notify, in writing, the
Regional Supervisor of the start-up(see permit for details).
6) 112R Status
Unknown at this time. Facility has not been constructed as of yet.
7) DAQ Compliance History
This facility has not started operations since its initial permit; it has had no prior DAQ violations.
8) Comments and Compliance Statement
This facility has not been established on site as of the date of this inspection. FRO has been informed
that the company still plans on developing the site sometime in the future.
BV Hedrick Gravel and Sand-Lilesville appeared to be in compliance on 20 June 2013.
