Dear Mr. Searles,
Your current air permit, identified above, includes Stipulation A.7 which requires that a
recurring report be sent to this office no later than 30 July 2014. Your facility failed to comply with
this requirement. Once the report is received in our office, a compliance determination, based upon
the content of the report, shall be made.
_ Please note that this is the third instance of air permit reporting failures by your facility
in four years. Please provide the subject report as soon as possible, and also provide to this office,
no later than 4 September 2014, a written response detailing the steps you will take to ensure you
meet these permit requirements in the future. Additionally, the Division strongly recommends that
you carefully examine your current permit stipulations.
