Dear Mr. Skrzypczak:
Thank you for your timely submittal of the subject report, received July 31, 2014. The excess
emissions report assessed the carbon monoxide (CO) continuous emissions monitoring systems
(CEMS) operating at the Haw River Facility during the first half of 2014. The Division of Air
Quality (DAQ) has completed its review and offers the following comments:
1. Hospital, medical and infectious waste incinerators(HMIWI) ES01 &ES02 were reviewed for
compliance with a state-only CO emissions limit of 11 ppmvd(permit condition 2.LA.La) and
a federal CO emissions standard of 40 ppmvd (permit condition 2.LA.1.h). Emissions were
corrected to 7% 02 and calculated on a 12-hour rolling average. All excess emissions were
attributed to either startup, shut down or malfunction. These emissions were reviewed and
allowed in accordance with applicable emission rules.
2. Quarterly summary reports were reviewed for compliance with good operation and maintenance
(O&M)practices pursuant to 40 CFR 60.11(d), General Condition 3.F and summarized as
follows:

The incinerators, along with associated CEMS units, appear to have operated using good O&M
practices since quarterly percent excess emissions (%EE)and percent monitor downtime

(%MD) were less than 6% for any single calendar quarter and less than 3% for two consecutive
quarters.
3. For both incinerators, CO CEMS cylinder gas audits (CGA) results were reviewed with respect
to specifications set forth in Section 5 of 40 CFR Part 60, Appendix F. The quality assurance
(QA)results appear to have met the 15% accuracy specification.
