Dear Mr. Skrzypczak:
The Division of Air Quality (DAQ) has reviewed the subject certification report received January
6, 2012, for the following carbon monoxide (CO) and oxygen (02)analyzers operating on
hospital, medical and infectious waste incinerator (HMIWI) ESO1 and HMIWI ES02:

Certification for each CO/02 continuous emissions monitoring system (CEMS) included a relative
accuracy test audit (RATA) and a 7-day calibration drift test. RATA testing was conducted by
Custom Stack Analysis, LLC, and 7-day drift tests were performed by Stericycle. Based on our
review, we offer the following comments:
1. For ES01, the RATA was performed on November 16, 2012 and the 7-day calibration drift test
was conducted from November 2, 2011 to November 8, 2011.

2. For ES02, the RATA was performed on November 15, 2012 and the 7-day calibration drift test
was conducted from November 2, 2011 to November 8, 2011.
3. Test results from the RATA indicate all CO and 02 analyzers appear to have met the relative
accuracy requirements set forth in Performance Specifications 3 & 4, which are found in 40
CFR Part 60, Appendix B. Since each analyzer also met its respective 7-day drift limit, ESO1
is certified effective November 16, 2012, and ES02 is certified effective November 15, 2012.
4. 40 CFR Part 60, Appendix F (permit condition 2.LA.l.h.)requires that quarterly audits must
be conducted, with a RATA being conducted at least once every four calendar quarters. The
next RATA for ESO1 and ES02 should be completed by December 30, 2012.
5. In order for DAQ to adequately assess the ongoing performance of the CO/02 CEMS
operating on ESO1 and ES02, we ask that your facility provide quarterly summaries of
percent excess emissions (%EE) and percent monitor downtime (%MD). Please note that
each summary should be calculated on a quarterly basis. DAQ considers a facility to have
operated with improper operation and maintenance (O&M) practices if the quarterly %EE
and/or %MD exceeds 6% for any single calendar quarter or 3% for two consecutive quarters.
We request that you provide the quarterly summaries on a semiannual basis in accordance
with 15A NCAC 02D .0605 and condition 2.1.A.Ld. of your air permit.
