Dear Mr. Melton:
Your written request for a name change for the Boggs Paving, Inc. facility in Lilesville,Anson
County, was received by this Division on January 30, 2015. This application submittal did not contain
all the required elements for processing. Since that date,both Gregory Reeves and Jeffrey Cole of this
office have spoken numerous times with your facility personnel in an attempt to have you submit the
proper forms and fees for this permit modification. Your application has been deemed to be incomplete
for processing and is being returned to you. As you are aware, Permit Stipulation B.5 states "This
permit is nontransferable by the Permittee. Future owners and operators must obtain a new air permit
from the DAQ".
Based on the information learned, it would appear that your request is for both a name change
and an ownership change for the facility. In order to process this request, air permit application forms
AA,AA3, and AA4 must be properly completed and signed by the respective officials and submitted
to, along with a check in the amount of$50 payable to NCDENR for the applicable processing fees for
the ownership change.
Please submit a written response to this letter 31 August 2015 and include a signed application
indicating the date of an ownership change . If you have questions regarding this application,please
