Dear Mr. Burrow:
Air Permit No. 06503RI I was issued to Greif Packaging, LLC—Southeastern Packaging, 2200
Mulberry Road, Concord, Cabarrus County,North Carolina on April 8, 2015, with an effective date of
April 8,2015. Specific Condition and Limitation A. 7. "New Source Performance Standards" (NSPS) of
said permit states in part:
a. ii. NSPS Reporting Requirements - . . . the actual date of initial start-up of an affected source
(ID No. ES-135),postmarked within 15 days after such date. If the affected source is
permitted to burn multiple fuels,then the actual date of start-up,for each fuel,must be
submitted and postmarked within 15 days after such date.
Ms. Donna Cook performed an inspection of your facility on May 3,2017. During this inspection,
a letter dated February 8, 2017 was provided to Ms. Cook indicating the boiler(ID No. ES-135) was
started on No. 2 fuel oil on July 26, 2016.
The submittal of the start-up notification for the boiler(ID No. ES-B5)was not postmarked within
15 days after the actual start-up of the boiler on No. 2 fuel oil. Therefore,this Notice of Deficiency is
issued to Greif Packaging, LLC—Southeastern Packaging for failure to submit the initial start-up
notification within the required time frame in violation of Air Permit No. 06503R11, Specific Condition
and Limitation No. A. 7. a. ii. "NSPS Reporting Requirements" and 15A NCAC 2)D .0524 "New Source
Performance Standards" (NSPS) as promulgated in 40 CFR Part 60, Subpart Dc.
