Dear Mr. Skrzypczak:
On January 30,2019,Taylor Hartsfield received the Annual Compliance Certification(ACC)
report for the above referenced facility. Upon review of the ACC report,Ms.Hartsfield discovered that
the annual hospital,medical,and infectious waste incinerator(HMIWI) operator training was not
performed in 2018. Therefore,you are deficient in fulfilling the requirements of 15A North Carolina
Administrative Code(NCAC)02D .1206 "Hospital,Medical,and Infectious Waste Incinerators"as
referenced in Condition 2.1.A.2.ee of Air Quality Permit 05896/T25,which states in part, "The Permittee
shall establish a program for reviewing this information annually with each HMIWI operator."
Please advise this office in writing no later than March 5,2019 of the cause of this deficiency,
what was done to rectify the situation,and what will be done to ensure that this type of deficiency does
not occur in the future. Additionally,this office recommends that you or a designated company
representative review your air permit to ensure that you understand all of the specific and general
permit conditions.
Failure to fulfill the requirements of Permit Condition 2.1.A.2.ce in the future may result in a
violation. All air quality violations are subject to enforcement action, including a civil penalty pursuant to
North Carolina General Statute(NCGS) 143-215.114A or permit revocation as authorized by NCGS 143-
