HomeMy WebLinkAbout19970299 Ver 1_CAMA Major Permit #109-84 Transfer_20190805Permit Class TRANSFER STATE OF NORTH CAROLINA Department of Environmental Quality and Coastal Resources Commission Vermt"t for X Major Development in an Area of Environmental Concern pursuant to NCGS 113A-118 X Excavation and/or filling pursuant to NCGS 113-229 Permit Number 109-84 Issued to Wayfarers Cove Marina & Beach, LLC., 4996 Indiana Avenue, Winston-Salem, NC 27106 Authorizing development in Pamlico County the Neuse River, Alligator Gut , as requested in the permittee's application letter dated 6/20/19. This permit, issued on August 6, 2019 , is subject to compliance with the application (where consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may Je suD3ect to renes, imprisonment or crvu acuon; or may cause me permit ro De nun ana vola. 1) This transfer must be attached to copies of the following permit documents, which includes all application forms and workplan drawings: a. The New Permit 109-84, issued to Keith A. Wolfenbarger on 6/18/84; b. The Minor Modifications issued on 4/12/85,4/22/88, and 5/20/88; c. The Renewals issued on 4/3/89, 4/23/90, 2/2791, 3/29/95, 3/29/00, 6/7/05, 2/25/15; d. The Major Modification issued on 8/13/07. 2) Copies of all referenced documents shall be readily available on site when a Division representative inspects the project for compliance. In accepting this transfer, the new permittee agrees that all conditions and stipulations of the active permit remain in force. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty (20) days of the issuing date. This permit must be accessible on-site to Department personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. All work must cease when the permit expires on December 31, 2019 In issuing this permit, the State of North Carolina agrees that your project is consistent with the North Carolina Coastal Management Program. Permit # 109-84 Page 2 of 2 ADDITIONAL CONDITIONS Maintenance Clause 3) The Division of Coastal Management shall be notified in writing at least two (2) weeks in advance of any maintenance excavation work authorized by this permit, and such notification shall include: A. The number of the original permit. B. A statement that no dimensional changes are proposed. C. A copy of the original permit plans with cross -hatching indicating the area to be maintained, the area to be used for spoil disposal, and the estimated amount of material to be removed. The location, design and holding capacity of the spoil disposal site shall be approved by a representative of the Division prior to the initiation of any maintenance dredging activities. D. The date of map revision and the permittee's signature shown anew on the original plan. NOTE: A transfer application processing fee of $100 was received by DCM for this project.
