Dear Mr. Odom:
On June 20, 2003, Mr. Larry Frost and myself performed a site visit with you and Mr. Ball at the
Hidden Lakes Subdivision site. The following needs to be received by the Division in order to
complete the review of this project:
1. Detailed drawings (with proper scale) of the proposed crossings including profile and
dimension of the placement of the proposed culverts. Additionally, all details of any
constructed headwalls must be included along with any additional footage requested for any
impacts to waterways from these headwalls.
2. An overview of the South Fork Creek crossing detailing the rerouting of the road and
placement of the crossing in relation to the current location in order to address the existing
head cut.
Additionally, please note for your files, that the Division of Water Quality can no longer accept 401
applications, which are not complete in nature. The submission of a Sediment and Erosion
control plan drawing does not provide sufficient detail to review proposed 401 impacts.
Additionally, because these projects may pass through several hands, and be reviewed by
multiple staff, complete and detailed directions must be included.
