Dear Mr. Johnson:
The Division of Water Quality (DWQ) is in the process of updating information contained in a new computer
database known as BIMS (Basinwide Information Management System). This database will replace
approximately 50 separate databases that were used in DWQ offices Statewide. This database will enable DWQ
to process information more efficiently and accurately, and give our customers better service.
We are requesting that you complete and return the attached Operator Designation form by June 28, 2004. Your
facility requires Grade 3 Wastewater Treatment System or higher Operator in Responsible Charge and Grade 2
Wastewater Treatment System or higher Back-up Operator. You may designate only 1 ORC, but you may
designate multiple Backup Operators.
The ORC of your facility must hold a valid certificate of the same type as the system, and a grade that is equal to
or higher than the class of your system. The Backup ORC(s) must hold a valid certificate of the same type as the
system, and no more than 1 grade lower than the class of the system. Failure to designate a properly certified
ORC and Back-up Operator by the above date constitutes a violation of the pennit issued for this
facility.
