(2) 105 linear feet of 4-span bridge #364 over Big Creek including 640 linear feet of approach fill on SR 1947 in
Columbus County, North Carolina
Description of Activity.
(1) replace 91 linear feet of existing 3-span 24-foot wide bridge #363 with 115 linear feet of 4-span bridge on a slight
realignment to the southwest of the existing structure and widening of 1160 feet of bridge approach, impacting 0 01
acre of wetlands This will also include the construction of a temporary cofferdam at the center bridge bent Retaining
walls will be constructed on north side (301 feet) of SR 1947 west of the bridge and on the south side (529 feet) east of
the bridge,
(2) replace 90 linear feet of existing 3-span bridge #364 with 105 linear feet of 4-span bridge on new alignment located
approximately 20 feet to the west of the existing structureand widening of 640 feet of bridge approach, impacting 0 17
acre of wetlands and 0 057 acres of surface waters Retaining walls will be constructed on the east (215 feet) and west
side (I 10 feet) of SR 1947 south of the bridge Traffic will be maintained on one lane of the existing and new bridges
during construction

This verification and will remain valid until the expiration date identified below unless the nationwide authorization
is modified, reissued or revoked If, prior to the expiration date identified below, the nationwide permit
authorization is reissued and/or modified, this verification will remain validuntil the expiration date identified
below, provided it complies with all modifications If the nationwide permit authorization expires or is suspended,
revoked, or is modified, such that the activity would no longer comply with the terms and conditions of the
nationwide permit, activities which have commenced (ie , are under construction) or are under contract to
continence in reliance upon the nationwide permit, will remain authorized provided the activity is completed within
twelve months of the date of the nationwide permit's expiration, modification or revocaton, unless discretionary

GENERAL PERMIT SPECIAL CONDITIONS
1 No in-water construction shall be conducted from April 1 through June 30 of any
year, which is during the spawning season for the Waccamaw silverside
2 Bridge demolition and removal will be accomplished in accordance with "Design
Standards in Sensitive Watersheds" and a copy will be provided to the contractor prior to
the pre-construction conference
3 No bridge demolition debris will be intentionally allowed to enter the waterway or
wetlands
4 Sediment and erosion control measures shall be in place prior to any demolition or
land clearing activities
5 Staging areas or the stockpiling of construction material shall not be located in any
wetlands
6 Both bridges shall be designed to exclude the use of deck drains
7 Measures will be taken to prevent live or fresh concrete, including bags of uncured
concrete, from coming into contact with waters of the state until the concrete has cured
8 All temporary fill is to be placed on filter fabric and removed from the waterway in its
entirety upon completion of the construction and the affected areas returned to pre-
construction elevations The affected vegetated areas must be replanted with native
vegetation at the completion of construction
9 All construction spoil material shall be disposed of on an upland area and shall not be
placed in any waterway or wetland
10 The permittee shall schedule a preconstruction meeting between its representatives,
the contractor's representatives, DWQ and the Corps of Engineers, Mr Richard Spencer,
Wilmington Regulatory Field Office, prior to any work within jurisdictional waters and
wetlands to ensure that there is a mutual understanding of all of the terms and conditions
contained within this Department of the Army Permit The permittee shall notify the Corps
of Engineers Project Manager a minimum of thirty (30) days in advance of the scheduled
meetings in order to provide that individual with ample opportunity to schedule and
participate in the required meetings One copy of the final half-size construction drawings
shall be furnished to the Corps of Engineers, Mr Richard Spencer, Wilmington Regulatory
Field Office prior to the pre-construction meeting
