Authorization Re-Issuance for the replacement of Bridge No. 63 over Middle
Creek on US 401, Wake County. Federal Aid Project No. BRSTP-401(13), State
Project No. 8.1404501,WBS Element No. 33350.1.1, Division 5, T.I.P. No. B-3916.

Reference: B-3916 Permit Application dated March 20, 2007 and 401 Water Quality
Certification 3627 and Neuse Buffer Authorization dated March 23, 2007.

Dear Sir:

The North Carolina Department of Transportation has been issued a Water Quality Certification
3627 and Neuse Riparian Buffer Certification from the North Carolina Division. of Water 1'41ality
(NCDW(?) for B-3916. These permits will expire when the associated US Army Corp of
Engineers (USACE) General Permit 31 expires on August 31, 2008.
USACE has reissued permits for this project using the Nationwide 23 a,~ n therefore NICDO'C
requests the corresponding Water Quality Certifications 3701 and 3688 and Neuse Riparian
Buffer Certification. NCDOT will comply with all conditions, descriptions, and mitigation
allowance in the previously submitted permit application dated March 20, 2007, PCI\ form;
pen-nit drawings, and EEP mitigation acceptance letter.
