The Division of Water Resources (DVVR) received an ernGi| from Kevin B0vvnn8n with the Federal
Energy Regulatory Commission on September 21, 2015 indicating that an application for a 401
Water Quality Certification should have been submitted t0 the Division. To date, the Division has
not received anapplication. Please note that the following must be received prior tO issuance ofa
4OI Water Quality Certification.
Additional Information Requested:
The 401 Certification cannot be processed until two (2) complete sets of the application and
associated maps are received at the DVVR Central Office in Raleigh along with the
appropriate fee.
Any large scale maps that are provided also need to include a copy of the site plans on a
Cd. 0n2(1) data CID 0f full size plans in TIFF Group 4 format (black and white, not gn3ysca|e
OrCO|Or\.|f the plans are too large tD store in TIFF format, they can be stored inPDF. If you

Application Fee: The fee for applications is $57O for projects innpaCtingoneOrnnore8[r2Sof
wetland or 150 linear feet of streams (whether intermittent or perennial).
