Mr. Jim Triplett Monroe Bypass Constructors, LLC 5562 Pendergrass Boulevard Great Falls, SC 29055 SUBJECT: Pre-Construction Conference Minutes A pre-construction conference was held Tuesday, June 9, 2015 for the above referenced project to be constructed by Monroe Bypass Constructors, LLC (MBC). See the attached list for meeting attendees. Safety • The entire project will be a “hard hat project”. Hard hat is required for all personnel on the jobsite. • Safety emphasis on trenching and shoring, backup alarms, crane safety, and fall protection. • MBC’s HS&E plan has been reviewed and approved by NCDOT. • Richard Thompson is the Division Safety Engineer and should be notified of all project safety issues. Right of Way • Sections 3A and 3B are clear except for Parcel 794. • Four parcels in 3A are subject to condemnation and will be filed by 6/15/15 (Parcels 682, 683, 685 & 687); these parcels will be clear to work on after June 15th. • Sections 2A and 2B are not all clear. ROW will be making a spreadsheet detailing the status of all properties for the project and their ROW status by the end of next week. • There are five known parcels (10, 15, 41, 48, and 91) with hazardous materials. Once these 5 parcels are acquired, NCDOT will mitigate contaminated material. Monroe Bypass Constructors Preconstruction Minutes July 6, 2015 Page 2 of 6 • 7 parcels (115, 216, 439, 345, 526, 102, and 16) are being re-evaluated for hazardous materials. If any are found, NCDOT will mitigate contaminated materials. • NCDOT policy is to work with property owners that have willingly granted ROW. If condemnation is required, contact with the property owners is limited to the legal department. USACE Permit – The following items were discussed and specifically outlined regarding the US Army Corps of Engineers Section 404 Individual Permit and NC Division of Water Resources Water Quality Certification: • Dust suppression measures will be implemented to reduce dust generated by construction when the control of dust is necessary for the protection of motorists and residents. • NCTA will coordinate with Mecklenburg County and Union County schools to share information to minimize impacts to school bus routes. • Two known populations of Schweinitz’s sunflowers will be staked and fenced off. The plants will be identifiable from May-November. Sunflower surveys need to be done between July and November at any potential offsite borrow waste site. o Meeting on-site with Fish and Wildlife to be scheduled within a few weeks to help stake and delineate sunflowers • Seeding will be required within 14 calendar days of completing construction activities in an area. • For any construction staging, storage, refueling, borrow pit or spoil area that is considered within the Goose Creek or Sixmile Creek watersheds, the NCDOT will coordinate with the USFWS, NCDOT Division Environmental Officer, and the contractor to determine if BMPs can be implemented for each site that avoid/minimize the potential for adverse effects to listed species and critical habitat. • A turbidity water quality testing program for the main stem of Stewarts Creek will be implemented to evaluate the performance of BMPs. Testing will be completed upstream and downstream of the construction area, as well as before, during, and after storm events. o MBC noted that turbidity testing started at the end of March. • The time limit for completing the work authorized by the permit ends on December 31, 2025. • Except as authorized by this permit or any USACE approved modification to this permit, no excavation, fill, or mechanized land-clearing activities shall take place at any time in the construction or maintenance of this project, within waters or wetlands, or shall any activities take place that cause the degradation of waters or wetlands. Special Conditions Borrow and Waste: • The permittee shall install barrier fencing around all wetlands that are not to be disturbed to make them readily visible and prevent construction equipment from inadvertently entering or disturbing these areas. Monroe Bypass Constructors Preconstruction Minutes July 6, 2015 Page 3 of 6 o MBC noted that all wetlands have been delineated with orange fencing in Segment 3. • If any construction staging, storage, refueling, borrow pit, or spoil areas are chosen within the Goose Creek or Sixmile Creek watersheds, the permittee shall coordinate with NC DOT Division Environmental officer and a representative from the US Fish and Wildlife Service and the contractor to develop Best Management Practices for each site to avoid and or minimize the potential for adverse effects resulting from such activity. Culverts: • Unless otherwise requested in the applicant's application and depicted on the approved work plans, culverts greater than 48 inches in diameter will be buried at least one foot below the bed of the stream. Culverts 48 inches in diameter and less shall be buried or placed on the stream bed as practicable and appropriate to maintain aquatic passage, and every effort shall be made to maintain existing channel slope. Violations: • Notify the NCDOT first of any permit violations. The NCDOT will notify the Corps of Engineers. Prohibitions on Concrete • The permittee shall take measures to prevent live or fresh concrete, including bags of uncured concrete, from coming into contact with any water in or entering into waters of the United States. Water inside coffer dams or casings that has been in contact with concrete shall only be returned to waters of the United States when it no longer poses a threat to aquatic organisms (concrete is set and cured). Notification of construction commencement • NCDOT shall notify the Corps in writing prior to beginning the work authorized by the permit and again upon completion of the work. o MBC noted they are ready to work in permitted areas as soon as they have been approved to do so. NCDOT emailed USACE a letter on June 9, 2015 notifying it intended to start permitted work immediately. Sediment Erosion Control • The permittee shall take measures to control any bottom sediment that may be sluiced by draining the pond(s) on the project site. Sediment sluicing from ponds is not authorized by this permit. • When draining or pumping water, control the pump rate. o No specific device or method required for dewatering. Design Schedule Monroe Bypass Constructors Preconstruction Minutes July 6, 2015 Page 4 of 6 • Section 2C RFC plans are scheduled to be complete and submitted in June. • Section 2B RFC plans are scheduled to be complete and submitted in late July. • Section 2A RFC plans are scheduled to be complete and submitted in August. • Segment 1 RFC plans are scheduled to be complete and submitted in September. • RK&K noted that there were no plan changes since USACE reviewed the plans near a permit site that would affect the USACE permit. • USACE requested that updated plans be sent in quarterly. • Permit drawings supersede plan drawings NCDENR Permit • Slopes of diversion ditches shall be sloped back and not vertical and be matted also. • Tall fescue shall not be used in the establishment of temporary or permanent ground cover within riparian areas. • Discharging hydroseed mixtures and washing out hydroseeders and other equipment in or adjacent to surface waters is prohibited. • DENR emphasized self-reporting and partnering on the project. MBC emphasized this as well. • MBC is required to monitor all rainfall on the project. • Weekly inspections of erosion and sediment control devices is required, or within 24 hours of notification of 0.5” rainfall in 24 hours. • MBC is responsible for keeping track of all changes, blowouts, or silt loss in the NPDES records. • Erosion and Sediment Control field reviews will be held on the first Tuesday of each month starting on July 7th. • Barge system is being proposed to be used for the construction of B2500. • There is no meeting required before entering each permit site. This preconstruction meeting will suffice. • RK&K and USACE are checking to see if baffles are required at Permit Site 16 (C1100). • C-2100 at Permit Site 35 needs to be buried 1 foot deep and not 6 inches as currently shown on design plans. • Permit Sites 19 and 20 need to meet planting requirements of the permit (riparian vegetation needed). Erosion Control • MBC to make a list of all certified Level II E&S certified individuals and submit to NCDOT. • Weekly punchlist to be prepared for everything that needs to be fixed. • Erosion and Sediment Control is required for all temporary work, even when it is not shown on the plans. • Ensure proper cleanup from vehicles tracking mud on the roadways. Ensure construction entrances are maintained. • MBC to have a copy of E&S plans on-site. Monroe Bypass Constructors Preconstruction Minutes July 6, 2015 Page 5 of 6 • All crews need a Level II certified E&S person and help ensure their work has BMPs installed. • Ground cover is key in E&S control. Stick to the specifications and rates for seeding. • GML and all other seeding contractors should avoid using annual rye grass on project. • Concrete washout sites are required. • Temporary mulch can be used as temporary ground cover but is not allowed to be incorporated into fill sections and must be removed. It also cannot be used in riparian buffer zones. Project Special Provisions • Supplemental Agreement No. 39 states that Substantial Completion Date is November 27, 2018 and Final Completion Date is May 28, 2019. The open road tolling intermediate contract date is 120 days before Substantial Completion Date. • ICT #4 has been revised; see Supplemental Agreement No. 41. • Certified payrolls are required on the project since it is a federally funded project. • DBE requirement is 15% of the original contract amount. o MBC stated that they will submit a new DBE utilization plan to NCDOT soon and they expect to meet the 15% goal. • If MBC utilizes DBE trucking as part of the project commitment, MBC must submit a trucking plan to NCDOT. NCDOT shall keep a daily trucking log for all DBE trucking companies. • MBC stated that they are tracking all DBEs whether or not they are on their commitment list. • No outsourcing outside of the USA for the project. • Three-Year Guarantee is included with this project. • Buy America will be followed for all materials on the job. • No burning of project material on- or off-site. • Pipe Installation and Pipe Culverts shall be performed in accordance with the Project Special Provisions. • Chemically-stabilized subgrades shall be tested for unconfined compressive strength for acceptance; lime-stabilized subgrades shall be tested using the dynamic cone penetrometer method and cement-stabilized subgrades shall be tested using field specimens. • The price for asphalt binder shall be adjusted per specifications. The base price index for asphalt binder for plant mix is $460.00 per ton. • A material transfer vehicle (MTV) shall be used when placing all full-width asphalt pavements. o MBC to submit letter requesting permission to not use a MTV on two-lane Y- lines. • Rock-blasting requires general site plan and site specific plan and requires meeting before each site blast. • Project will be using Constructware. Next round of training will be held soon. Box files will be used as a predecessor to filing in Constructware. • Inspectors will operate under the 2015 QMS. MBC to evaluate if this will cost them more money. Monroe Bypass Constructors Preconstruction Minutes July 6, 2015 Page 6 of 6 • Prior to installing guardrail anchor units, MBC shall submit the FHWA acceptance letter and the certified working drawings and assembling instructions to the NCDOT. • Final surface testing on Y-lines shall be performed using Hearne Straightedge. The mainline final surface smoothness shall use the international roughness index (IRI) with NCDOT providing the equipment, performing the testing, and then analyzing and evaluating the results. • A final lift meeting will be held before the placement of the final lift of asphalt. • Federal aid provision FHWA 1273 needs to be in all subcontracts. o MBC noted that they have put it in all subcontracts to date and will continue to do so. • Special attention should be made to ensure that the longitudinal joint is not underneath the pavement markings. • The Contract requires polyurea pavement marking lines with highly reflective elements on both –L- and Y-lines. MBC noted it plans to use cold applied plastic pavement marking tape on bridges. o NCDOT requests MBC evaluate the cost difference in placing thermoplastic pavement marking lines on the Y-lines. • Press release will go out after the E&S controls on 3B are done (within a month). • NCDOT and MBC agreed to delay the formation of the Dispute Review Board until it is needed. These minutes are prepared as they are documented. Any discrepancies with the content of these minutes should be submitted in writing to this office. Failure to do so will be an indication of agreement.
