On September 15, 2014, the Division of Water Resources (the Division) received your
application dated September 12, 2014 for the subject project. Staff has reviewed the application and
on September 23rd, a site visit was conducted with you and your representative, Mr. Tamp Bandy.
During the site visit, further information and clarification of the proposed project was requested. This
information is required to complete our review of this project:
The storm water condition of General Certification 3886 that corresponds with the
Nationwide Permit 14 must be met. A storm water plan is required for the new phase of
the project.
A diagram for the proposed culvert installation is required with the appropriate
information, pipe(s) placement and size, etc. Include cross section of the proposed
culvert.
Please be advised that this project will be placed on hold pursuant to 15A NCAC 2H .0507(h)
until the information listed above is provided. In addition, this Office is requesting that the Army Corps
of Engineers place a hold on processing the application. If you have any question, please contact Mr.
