Subject: Modification request for additional impacts at Site 2 associated with
secondary road improvements to SR 1432 (Crawford Currin Road) fi•otn
SR 1430 to SR 1435 in Granville County. WBS element SC.039033.
The secondary road improvements to SR 1432 (Crawford Currin Road) in Granville County
are currently underway. This paving project involves two permit sites. Please note that this
modification request specifically addresses additional impacts proposed at Site 2. Proposed
impacts at Site 1 have not changed. The construction plan on the Crawford Cutrin Road
consists of widening the original 15ft gravel roadway. The referenced application proposed
replacing this existing 36-inch reinforced concrete pipe at Site 2 that was 25 feet long with
headwalls with a pipe that is 60 feet long. This proposal has not changed. This construction
will provide a pavement width of twenty feet and four foot earth shoulders with appropriate
front and back slopes. The section of this road at Site 2 will require about 2 % feet of till
(raised grade). This fill height will be required to ensure that the appropriate cover
requirement is achieved for the 36-inch RCP installation and also to ensure that storm watec•
during an intense rain event does not wash over the roadway.
Recent right-of--way staking through this area has revealed that there will be stream impacts
beyond those originally reported (see attached picture). Due to the position of a section of
stream running along the existing toe of fill on the West side of the road, a total of 125 feet of
impacts are now proposed (68ft EXTRA). This total includes 16ft of new pipe length on the
outlet and l Oft of rip rap armoring, then 19ft of new piped length on the inlet, moving 80 feet
of stream, and armoring with rip rap as needed along the entire stretch. NCDOT proposes to
stabilize the moved channel and associated banks by revegetating this area and by using rip
rap only as needed. Any rip rap used for armoring will not impede with aquatic life passage.

Please note that the 2ft wide stream above the inlet of this cross line runs within the right-of-
way limits approximately 80ft before entering the pipe at an angle. Then the Oft wide stream
below the outlet flows straight for 17ft before taking a hard left at the right-of--way limits and
runs adjacent to the proposed construction limits for over 150ft. Additionally wetlands exist
surrounding the stream segment on the outlet end.
Avoidance and Minimization:
Complete avoidance of stream impacts at Site 2 is not possible. The 60ft of pipe proposed is
the minimum length allowed to provide an adequate two lane roadway width and safe
shoulders through this area. The 2:1 fill slopes proposed in this area cannot be steepened for
safety considerations. If the roadway was shifted to East approximately loft to avoid the
channel move (now proposed on the inlet), impacts to the outlet end alone would total 108ft
• of stream impacts and 0.02ac of associated wetland impacts.

Mitigation
The referenced USAGE General Permit Verification lists permanent authorized impacts of
37 feet (which was the piped length) and set the required mitigation of 74 linear feet In-Lieu
to EEP. Since the length of piped stream has actually decreased from 37ft to 35ft and the
stream segment to be moved should retain its biological function though an open channel,
NCDOT is not proposing mitigation for these additional impacts to this small weak-perennial
stream.
Summary
Please see the attached revised permit drawing and updated PCN for details on the revised
impacts at Site 2. Please note that the updated PCN includes these extra impacts at Site 2 and
presents the new grand total for the entire project. Permit modifications are hereby requested
for the referenced Department of the Army Section 404 Nationwide Permit (NWP) 14 and
NCDENR-DWQ Section 401 Water Quality Certification (WQC) No. 3404 and WQC No.
3366 for above-described activities. An automated payment procedure has been implemented
between the NCDOT and NCDWQ. This procedure will enable the Division to apply for this
modification without submitting a check. This procedure will provide payment to the
NCDWQ by charging the permit modification fee of $200.00 directly to the appropriate
NCDOT WBS element SC.039033. If you have any questions or need additional information,

No compensatory wetland mitigation is required.
The permanent loss of 371inear feet of stream associated with this project shall be mitigated by NCDOT by providing 74
linear feet of restoration equivalent warm water stream channel in the Roanoke River basin (Hydrologic Cataloging Unit
03010102). Based on your application, you intend to utilize the Ecosystem Enhancement Program (EEP) as
recommended in our letter dated September 1, 2004 from Mr. Ken Jolly to Dr. Gregory J. Thorpe. Until the Corps
receives a confirmation letter from EEP that EEP is willing to provide mitigation for this project, NCDOT is responsible
for this mitigation requirement. If the Corps has not received confirmation from EEP within 150 days of the date of this
verification, NCDOT shall provide plans to accomplish the required mitigation to the Corps of Engineers within 210 days
of the date of this verification.
Note: This verification does not include fill impacts to waters of the United States from any borrow sites, spoil disposal,
construction access, or on-site detours. These impacts would have to be permitted separately.

1. Placement of culverts and other structures in waters, streams, and wetlands shall be placed below the elevation of
the streambed by one foot for all culverts with a diameter greater than 48 inches, and 20 percent of the culvert
diameter for culverts having a diameter less than 48 inches, to allow low flow passage of water and aquatic life.
Design and placement of culverts and other structures including temporary erosion control measures shall not be
conducted in a manner that may result in dis-equilibrium of wetlands or streambeds or banks, adjacent to or
upstream and down stream of the above structures. The applicant is required to provide evidence that the
equilibrium is being maintained if requested in writing by DWQ. If this condition is unable to be met due to
bedrock or other limiting features encountered during construction, please contact the NC DWQ for guidance on
how to proceed and to determine whether or not a permit modification will be required.
2. Riprap should not be placed in the active thalweg channel or placed in the streambed in a manner that precludes
aquatic life passage. Bioengineering boulders or structures should be properly designed, sized and installed.
3. For any streams being impacted due to site dewatering activties, the site shall be graded to its preconstruction
contours and revegetated with appropriate native species.
4. If concrete is used during construction, a dry work area should be maintained to prevent direct contact between
curing concrete and stream water. Water that inadvertently contacts uncured concrete should not be discharged to
surface waters due to the potential for elevated pH and possible aquatic life and fish kills.
5. During the construction of the project, no staging of equipment of any kind is permitted in waters of the U.S., or
protected riparian buffers.
6. The dimension, pattern and profile of the stream above and below the crossing should not be modified.
Disturbed floodplains and streams should be restored to natural geomorphic conditions.
7. The use of rip-rap above the Normal High Water Mark shall be minimized. Any rip-rap placed for stream
stabilization shall be placed in stream channels in such a manner that it does not impede aquatic life passage.
8. All work in or adjacent to stream waters shall be conducted in a dry work area. Approved BMP measures from
the most current version of NCDOT Construction and Maintenance Activities manual such as sandbags, rock berms,
cofferdams and other diversion structures shall be used to prevent excavation in flowing water.
9. Heavy equipment shall be operated from the banks rather than in the stream channel in order to minimize
sedimentation and reduce the introduction of other pollutants into the stream.
10. All mechanized equipment operated near surface waters must be regularly inspected and maintained to prevent
contamination of stream waters from fuels, lubricants, hydraulic fluids, or other toxic materials.
11. No rock, sand or other materials shall be dredged from the stream channel except where authorized by this
certification.
12. Discharging hydroseed mixtures and washing out hydroseeders and other equipment in or adjacent to surface
waters is prohibited.
13. All fill slopes located in jurisdictional wetlands shall be placed at slopes no flatter than 3:1, unless otherwise
authorized by this certification.

14. A copy of this Water Quality Certification shall be posted on the construction site at all times. In addition, the
Water Quality Certification and all subsequent modifications, if any, shall be maintained with the Division Engineer
and the on-site project manager.
15. The outside buffer, wetland or water boundary located within the construction corridor approved by this
authorization shall be clearly marked by highly visible fencing prior to any land disturbing activities. Impacts to
areas within the fencing are prohibited unless otherwise authorized by this certification.
16. Upon completion of the project, the NCDOT Division Engineer shall complete and return the enclosed
"Certification of Completion Form" to notify DWQ when all work included in the 4C1 Certification has been
completed.
17. Native riparian vegetation must be reestablished within the construction limits of the project by the end of the
growing season following completion of construction.
18. There shall be no excavation from, or waste disposal into, jurisdictional wetlands or waters associated with this
permit without appropriate modification. Should waste or borrow sites be located in wetlands or streams,
compensatory mitigation will be required since that is a duect impact from road construction activities.
19. Erosion and sediment control practices must be in full compliance with all specifications governing
the proper design, installation and operation and maintenance of such Best Management Practices in
order to protect surface waters standards:
a. The erosion and sediment control measures for the project must be designed, installed, operated, and
maintained in accordance with the most recent version of the North Carolina Sediment and Erosion
Control Planning and Design Manual.
b. The design, installation, operation, and maintenance of the sediment and erosion control measures
must be such that they equal, or exceed, the requirements specified in the most recent version of the
North Carolina Sediment and Erosion Control Manual. The devices shall be maintained on all
construction sites, borrow sites, and waste pile (spoil) projects, including contractor-owned or leased
borrow pits associated with the project.
c. For borrow pit sites, the erosion and sediment control measures must be designed, installed, operated,
and maintained in accordance with the most recent version of the North Carolina Surface Mining
Manual.
d. The reclamation measures and implementation must comply with the reclamation in accordance with
the requirements of the Sedimentation Pollution Control Act.
20. Sediment and erosion control measures shall not be placed in wetlands or waters unless otherwise approved by
this Certification. If placement of sediment and erosion control devices in wetlands and waters is unavoidable, they
shall be removed and the natural grade restored upon completion of the project.
