Hi Dave. I wanted to let you know that the erosion control measures are repaired and offsite/in stream sediment has
been removed. Kat reviewed the site on Monday and I reviewed today. Let me know if you have any questions.

Dave,
Our contractor told us that they didn’t work on Monday or Tuesday because they considered it unsafe due to the wet
conditions. Since the permit is ultimately DOT’s responsibility, we initiated the inspection yesterday in the rain and the
contractor joined us in completing today.
We’ll let you know when the repairs have been completed.

Amy,
Let me know when all repairs are made to EC measures, and please provide reason(s) that explain why
inspections were delayed. Thanks.

Subject: R-2247EB sediment losses
We had 6 sediment losses on R-2247EB following rains received between Thursday and this morning. We still have one
culvert underwater and will let you know if there is anything to report when it subsides. The losses are as follows:
Site 9A-III had a loss due to a sliding of the slope and overwhelming the SDO approximately 9 CY
Site 10A-VV had a silt loss due to an SDO failure approximately 20 5 gal buckets.
Site 10B-VV had a silt loss due to failure of inlet protection around a box approximately 2 CY.
Site 13A-DDD had a silt loss due to an SDO failure approximately 20 5 gal buckets.
Site 15B-XX had a silt loss due to SDO failures approximately 1 CY.
Site 15G-XX had a silt loss due to SDO failures approximately 2 CY.
Our apologies for the delay in the inspections. Our prime contractor did not complete the NPDES inspections in the
required time. DOT began the inspection yesterday, and DOT and the prime contractor’s representatives completed the
NPDES inspections today. Our Resident Engineer’s office has shut down the prime contractor’s operations and any
subcontractor operations that disturb the ground until the repairs on the list are completed within the appropriate time
frames. The prime contractor began working on the erosion control measures today. Let me know if you have
questions or need additional information.
