All, First and foremost we want to assure NCDOT that FBDJV and Site-Prep are fully committed to cleaning up the area and preventing this incident from happening again. Below are the procedures that we are putting in place to make sure this does not happen again. 1. Site-Prep has reviewed all company policies with personnel and has already had a field meeting to convey how this process will be performed and why it is done this way with field personnel. 2. Truck washouts will be performed on the grade near the mixing operations when possible, when it is not possible the trucks shall wash out up hill from all low areas where jurisdictional streams are located and away from all slope drains and DI’s. 3. Where soil stabilization is occurring in the JS areas (lower areas) FBDJV will plug slope drains and boxes to not allow slurry to reach the drainage system or convey the slurry down to the EC measures already in place. As discussed in the field leaving the slope drains in place and only plugging them leaves the project team a way to convey stormwater to the appropriate BMP’s in the event the project was to get an afternoon thunderstorm. Below is a brief update on where we stand with cleanup operations.  All new mixing operations have been suspended until cleanup has been completed and accepted by all parties. Work is only taking place on areas that previously had product in the ground.  FBDJV and Site-Prep mobilized to the site yesterday and have cleaned the slurry off of the roadway and hauled off  All rock checks have been replaced and ditch lines cleaned of the washout slurry  Slurry that made it into the creek has been removed with buckets  Wattles were placed in the creek yesterday to prevent any further slurry loss  KRG has mobilized to the site today and will be cleaning the creek and drainage system with a vac truck. FBDJV has installed an impervious dike to not allow further offsite, KRG will haul water and slurry to other parts of the project and will be disposed of away from low lying areas, DI’s, and slope drains on the grade. When the spill happened FBDJV was able to obtain PH samples of the stream that was effected, below are our results. Once cleanup operations are completed we will retest the area again and report that data once it is completed. I’m not sure how you intend to clean it up, but a recommendation that I think would work would be to install a dike to prevent any water and residuals from going downstream. Flush the drainage system and rocks in front of the dike while using a vac truck at the same time to suck up all the water / slurry. Of course, the truck would have to be taken to an approved wastewater site. I’m not directing you on how to clean it up, only making a suggestion. All resources required to remedy this issue will be dedicated to it from both FBDJV and Site Prep. My understanding of it currently is that we have addressed everything on land and are awaiting direction on being allowed to clean the stream channel. Kristofer A. We are currently doing chemical stabilization of the grade on the U-2579DEF project. Our inspectors discovered a lime slurry spill this afternoon on the grade. We are currently trying to determine what happened. The lime solution went down the slope and into a basin. There is evidence of slurry in the stream. I have attached some pictures showing the lime slurry on the grade, in the basin, a film shown in the stream, and clean up measures underway. DOT has directed Flatiron to cleanup the slurry from the grade, basin, etc. We have instructed them to obtain pH samples upstream, and downstream at 100’ intervals to determine if the lime had an impact on the pH of the waters. We have also directed them to install a series of wattles in the stream to the limits of the staining, and collect any accumulated solids/slurry from the behind the wattles and anywhere along the stream that it has deposited. We’ll keep you up to date when we receive additional information. Note this is permit site 6 (basin 10.5) on the project. Let me know if you would like us to do anything additional.
