Attached is the draft concurrence meeting packet for TIP Project
R 2583 US 158 widening in Hertford County from the Murfreesboro
Bypass to US 13 You will receive the usual concurrence packet
two to three weeks before the January 17th scheduled Concurrence
meeting but the attached information is being provided prior to
our field meeting to allow you time to review NCDOT s
information I will provide paper copies of this document with
maps at the field meeting for your use
For the field meeting we will meet at the Mount Tabor Baptist
Church located in the middle of the project on US 158 The
meeting will begin at 9 30 with an introduction and project
overview From the Church we will go to the existing US 158
bridge over Potecasi Creek then to Mill Branch We will then
travel to the Alternative C crossing location of Potecasi Creek
and finally to the Mapleton Community to look at the crossing of
the Tributary to Potecasi Creek
If you have any questions after reviewing the attached document
please let me know otherwise we are prepared to answer questions
at the field meeting

Good fob[ I m all over this as well Same problems you have I have as well
I had a submittal form with mine that had some contacts and phone numbers with it Call
Khaled Al Akhdar (he s first contact listed) at 919 707 -6612
He s already got an earful from me They are in process of getting me signatures along with
some other stuff (EEP letter) as well As I got deeper into it as I m preparing my public
notice, I also picked up that the impact numbers in the cover letter do not match the impact
summary sheet numbers
So they are getting me a corrected cover letter as well as they have told me the numbers in
the summary sheet are the correct numbers Apparently NEU never got this application to
review before it was sent out It came directly from consultant and design build side of the
house Chris Manley is frustrated because I m going to him about the problems with the
application and he had nothing to do with it Sounds like DOT had a little internal miss
communication of how the process is suppose to work'

I have done a brief review of the application for R 2583, US 158 in Hertford County, and have
a few things that need to be addressed I am addressing this to you because there is no
contact listed in the application (dust says contact Eme0) Four concerns
1 The ENG Form 4345 is not signed
2 The CAMA permit is not signed
3 You list an agent (Arcades) but have not signed the ENG 4345 form
giving them permission to act on DOTOs behalf
4 There is no EEP acceptance letter attached

Since DWQ does not have its own form for IPs, we essentially piggyback on the others What we
require is a signature from the applicant accepting responsibility for the project Where an
agent is involved, we need acknowledgement that they have been given permission to act on
behalf of the applicant In this case both could be taken from the Corps form
Please address these deficiencies by the end of next Thursday (December 22) or we may return
your application as incomplete Should you have any questions, please let me know Thank you
