On November 3 2011 NCDWQ sent a letter placing the permit application for the above
reference pro�ect on hold for the following reasons
• The perenmal stream on plan sheet 6 for which impacts have not been accounted Please
update the impact table and mitigation total to account for impacts to this stream
• The US Army Corps of Engineers (USACE) has noted discrepancies m the NEPA
documentation Please work out this issue with the USACE and provide the Division of
Water Quality wrth copies of any correspondences
• A very small amount of the unvegetated stream at Site 4 will remain open between two
pipes after construction This channel should be stabilized and considered a total take
Please update the impact tables and mitigation totals to reflect this loss of stream channel

Since the on hold letter was received the USACE has resolved the NEPA issues through phone
conversat�ons with the NCDOT Natural Environment In addition an application update memo
dated December 7 2011 has been prepared and distributed that addresses the first and last bullets
in the on hold letter
Therefore we believe NCDOT has satisfied the deficiencies in the origmal August 24ih 2011 IP
application and that permitting can proceed
