Department of Transportation 253 Webster Road Sylva, North Carolina 28779 Subject: Inspection NCSR 1783 Upward Road DWQ Project 08-1327 TIP No. R-4430 Henderson County Dear Mr. Setzer: On August 4, 2011, I inspected the construction work on Upward Road between US Highway ] 76 and Howard Gap Road in Henderson County. Construction work is continuing all along Upward Road between Howard Gap Road and US Highway 176. Please ensure that all sediment and erosion control structures are being maintained as required and that seeding and mulching are being performed as needed, to protect State surface waters. Construction is underway all along the project. 2. At 72" pipe junction, install bens at top of slope to prevent water from washing silt into stream. 3. Clean out sediment below the outlet of the 48" RCP and refresh #57 stone and stabilize the slope on the north side of Upward Road at 48" RCP with matting and rip rap to prevent material from washing into stream. 4. Install rock fore bay at the stortnwater pond to prevent silt from spreading across the pond. 5. Continue to maintain all sediment and erosion control devices and seed, mulch and matt all disturbed areas when needed to protect State surface waters. Upward Road R-0430 Inspection August 201 I
